Summary

Main use
Treatment of herpes zoster and genital herpes, and suppression of recurrent genital herpes
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: Famciclovir is administered orally at 250 mg per dose 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: This drug is administered orally at 250 mg per dose 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: This drug is administered orally at 125 mg per dose twice daily for 5 days. It is preferable to start treatment during the prodromal phase or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: This drug is administered orally at 250 mg per dose twice daily. Treatment should be interrupted periodically every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: Because the clearance of this drug is reduced in patients with renal impairment, dose adjustment should be undertaken with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose twice daily 10~29 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59 125 mg per dose twice daily 10~29 125 mg per dose once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose twice daily 10~29 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients undergoing hemodialysis, administration every 48 hours is preferable, taking the interval between dialysis sessions into account. Four hours of hemodialysis reduces the plasma concentration of this drug by approximately 75%; therefore, administer this drug immediately after dialysis. The recommended dose is 250 mg for patients with herpes zoster and 125 mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.

Caution is required when administering this drug to patients with impaired renal function, and dose adjustment may be necessary as needed. This drug is generally well-tolerated, with symptoms like headache or nausea being mild to moderate, but usually not serious in most cases. It can also be safely used in immunocompromised patients, but rarely, blood-related adverse reactions such as thrombocytopenia or leukopenia may occur, so caution is warranted. Gastrointestinal side effects such as nausea and vomiting have been reported rarely, and liver dysfunction or jaundice may also occur infrequently. Skin reactions such as rash, itching, urticaria, and severe skin reactions may rarely occur. In patients with decreased renal function, serious renal issues, such as acute renal failure, can infrequently occur, so dose adjustments should be made based on renal status. This drug does not completely cure genital herpes, and patients should be educated to avoid sexual contact while symptomatic and to start treatment immediately when recurrence symptoms appear. There is no evidence that it affects driving or operating machinery; however, if dizziness, drowsiness, or confusion occur during use, it is advisable to avoid such activities. Patients with liver dysfunction can use this medication without special dose adjustments if renal function is normal. No clinically significant drug interactions have been reported with this drug; however, caution is advised as blood levels may change when combined with certain medications. Safety during pregnancy has not been established; it should be administered only if necessary at the discretion of a physician, and nursing mothers should only use this drug when its benefits outweigh risks. In children under 18 years old, safety and efficacy have not been fully established, so it is preferable to avoid administration. In case of overdose, there may be no specific symptoms, but appropriate treatment and blood dialysis may be required to lower drug concentration if necessary.
