Summary

Main use
Reduction of platelet count in myeloproliferative disorders, reduction of thrombotic risk, and improvement of symptoms of thrombosis and bleeding
Who takes it
Adults · Children
Adult dose
The initial dose for adults is 0.5mg four times daily or 1.0mg twice daily, with close monitoring, and should be maintained for at least 1 week. For both adults and children, administer the lowest effective dose needed to reduce the platelet count and maintain it at or below 600,000/μL; keep the platelet count within the normal range as far as possible. When increasing the dose, the increase should be no more than 0.5mg/day, and the dose should not be increased again within 1 week. The maintenance dose is the same for adult and pediatric patients. Do not exceed 2.5mg as a single dose or 10mg as the total daily dose. To monitor the effect of this medicine and prevent the development of thrombocytopenia, check platelet counts every 2 days during the first week of treatment and at least once weekly after reaching the maintenance dose. In general, the platelet count begins to decrease within 7~14 days at an appropriate dose. It takes 4~12 weeks to achieve a complete response, defined as a platelet count at or below 600,000/μL. Most patients will respond adequately at doses of 1.5~3.0mg daily. Carefully monitor patients with a history of heart disease, renal failure, or hepatic impairment, or in whom these conditions are suspected.
Child dose
The initial dose for children ranges from 0.5mg daily to 0.5mg four times daily. Based on limited data, an initial dose of 0.5mg daily is recommended for children.

View approved original text

This medication is used for patients with myeloproliferative disorders such as essential thrombocythemia, polycythemia vera, and chronic myelogenous leukemia, who have an abnormal increase in platelet counts. This drug effectively reduces elevated platelet counts and lowers the risk of thrombosis. It also improves related symptoms such as thrombosis and bleeding tendencies, aiding the overall health of patients. Thus, it plays an important role in the prevention of various complications and alleviation of symptoms caused by increased platelets.

How to take

The initial dose for children ranges from 0.5mg daily to 0.5mg four times daily. Based on limited data, an initial dose of 0.5mg daily is recommended for children.

Precautions

This medication should not be used in patients with severe hepatic impairment because its concentration in the body can increase significantly in patients with severely decreased liver function. Additionally, it should not be administered to patients with genetic diseases who cannot break down lactose, as those patients may experience problems due to the lactose included in this medication. Use this medication cautiously in patients with or suspected of having heart disease, as it may increase heart rate and contractility, putting a burden on the cardiovascular system. It is important to conduct heart-related examinations in advance and to continuously monitor the heart condition during administration. Caution is needed as changes in heart rate or electrocardiogram may occur. Patients with slightly impaired liver function also require careful evaluation before starting this medication, and liver function and cardiovascular status should be closely monitored along with dose adjustments. Patients with lung disease or pulmonary hypertension should also be cautious when using this medication, as it may cause pulmonary-related side effects. During administration, side effects like headaches, palpitations, fluid retention, nausea, and diarrhea may frequently occur. Gradually adjusting the dosage can help reduce side effects. Regular blood tests should be conducted to monitor platelet counts, as significant fluctuations may occur. Sudden discontinuation of the medication should be avoided, as it can lead to a rapid increase in platelet counts and a risk of thrombosis. When used concurrently with medications affecting blood coagulation such as aspirin, the risk of bleeding may increase; therefore, careful evaluation of risks and benefits is essential. Women who are pregnant or may become pregnant should not use this drug, and it should be discontinued immediately if pregnancy is confirmed during use, due to potential harm to the fetus. Breastfeeding women should also consider stopping breastfeeding or discontinuing the medication as it may affect the baby. Experience with pediatric patients is limited, but similar side effects as in adults may occur, requiring caution. Elderly patients may have a higher incidence of cardiac-related side effects, necessitating even more careful monitoring. In cases of overdose, thrombocytopenia and cardiovascular and central nervous system side effects may occur; therefore, if overdose is suspected, immediate treatment under medical supervision is necessary. This medication should be stored out of reach of children, and transferring it to another container is not advisable for quality maintenance and safety.
