This medication is used to enhance blood sugar control in patients with type 2 diabetes. It can be administered alone as an adjunct to dietary management and exercise, and is effective even when used in combination with oral hypoglycemic agents or insulin when blood sugar control is difficult. Additionally, when blood sugar control is inadequate with monotherapy using sulfonylureas or metformin, it is administered in combination with metformin to improve blood sugar management. This medication helps to stabilize blood sugar levels, thus aiding the patient's health.

How to take

Adult dose

This medication is taken orally once a day. It should be taken before breakfast or immediately before the first meal. Initiate treatment with 1 mg of glimepiride once daily, and if necessary, the dosage can be increased by 1 mg at intervals of 1 to 2 weeks. Generally, dosages exceeding 4 mg provide minimal additional effects; however, in some cases, dosage can be increased up to 6 mg or 8 mg. A daily dosage of 4 mg is considered effective.

Precautions

This medication should not be administered to patients with insulin-dependent diabetes or diabetic ketoacidosis. Patients who have had allergic reactions to this medication or its components should not use it. In patients with severe liver or kidney function impairment, switching to insulin therapy is recommended instead of using this medication. Women who are pregnant or may become pregnant, as well as nursing mothers, should not take this medication. Patients with severe infections, pre- or post-surgery, or significant trauma are advised to use insulin therapy. Particularly, elderly patients, those with poor nutritional status or irregular eating habits, and individuals who consume alcohol frequently are more susceptible to hypoglycemia. Caution is also required if there is an imbalance in physical activity and carbohydrate intake or changes in meal patterns. Patients with weakened kidney or liver function or those who have overdosed on other medications also have an increased risk of hypoglycemia. Also, elderly patients with endocrine disorders such as thyroid abnormalities or adrenal insufficiency, and those on certain medications such as beta-blockers, should be monitored carefully. Additionally, careful observation is necessary when used in combination with drugs that could affect blood sugar control. This medication may cause symptoms of hypoglycemia. Symptoms of hypoglycemia include headache, severe hunger, nausea, fatigue, anxiety, decreased concentration, confusion, drowsiness, and in severe cases, loss of consciousness or coma. In cases of hypoglycemia, it is crucial to immediately consume food containing sugar or seek medical assistance for treatment. Temporary blurred vision, gastrointestinal disturbances, liver dysfunction, blood abnormalities, or allergic reactions may occur during the initial stages of treatment. Consultation with a physician is necessary if any adverse symptoms occur. It is vital to regulate blood sugar well by taking this medication regularly along with dietary and exercise programs. To prevent hypoglycemia, always keep food containing sugar on hand, and be aware of hypoglycemia symptoms and treatment methods. In situations of stress, surgery, or infection that make blood sugar control difficult, temporary switching to insulin therapy may be necessary. Regular blood sugar and urinary sugar tests should be performed to verify medication effectiveness and adjust treatment methods as needed. This medication is excreted through the kidneys, so elderly patients or those with decreased kidney function should take care with dosage adjustments. In case of overdose, severe hypoglycemia may manifest, and appropriate treatment should be sought immediately at a medical facility.

Serious side effects

Hives, severe swelling (edema), difficulty breathing, hypotension, shock, skin and mucosal syndrome (Stevens-Johnson syndrome), anaphylaxis, angioedema, liver dysfunction (jaundice), severe allergic reaction, severe hypoglycemia, loss of consciousness, coma, shallow breathing, cerebral convulsions, seizures, thrombocytopenic purpura, severe blood abnormalities, platelet counts below 10,000/μL, liver failure, and clinical symptoms of severe hypoglycemia or severe edema.

Mild side effects

Headache, severe hunger, nausea, vomiting, fatigue, sleep disorders, anxiety, aggression, decreased concentration, decreased agility or activity, depression, confusion, speech impairment, aphasia, visual disturbances, tremors, incomplete paralysis, sensory abnormalities, dizziness, weakness, loss of self-control, transient mental confusion, drowsiness, bradycardia, sweating, cold and moist skin, tachycardia, hypertension, palpitations, angina pectoris, deep vein thrombosis, blurred vision, or fullness or pressure in the upper abdomen.
