Who takes it
Adults
Adult dose
Do not increase the dose or stop taking this medicine without your doctor's instructions. 1. Essential hypertension 1) Adults (1) Administer 12.5 mg of carvedilol orally once daily for the first two days, then administer 25 mg orally once daily. (2) The usual dose is 25 mg orally once daily for 2 weeks. If an increase is needed, increase to 25 mg twice daily. The maximum dose per administration is 25 mg, and the maximum daily dose is 50 mg. 2) Elderly patients In some elderly patients, 12.5 mg once daily may provide an adequate response. However, if the response is inadequate after 2 weeks of treatment, the dose may be increased. 2. Chronic stable angina 1) Administer 12.5 mg of this medicine twice daily for the first two days, then administer 25 mg twice daily. 2) The usual dose is 25 mg twice daily for 2 weeks. If an increase is needed (excluding elderly patients), the dose may be increased to 50 mg twice daily. 3. Congestive heart failure Before starting treatment with this medicine, the patient's existing standard treatment regimen (conventional treatment for chronic heart failure with an ACE inhibitor, a diuretic, and usually a digitalis preparation) must be stable. Fluid retention should also be minimized before starting treatment with this medicine. 1) The usual initial dose is 3.125 mg of this medicine orally twice daily for 2 weeks. If the patient tolerates this treatment well, increase the dose to 6.25 mg twice daily for 2 weeks, then to 12.5 mg twice daily for 2 weeks, and then to 25 mg twice daily. 2) After taking each dose for at least 2 weeks, gradually increase the dose to the highest dose the patient can tolerate. 3) The effective minimum dose is 6.25 mg twice daily. The maximum dose is 25 mg twice daily for patients weighing 85 kg or less, and 50 mg twice daily for patients weighing more than 85 kg. 4) To control the rate of absorption, it is recommended that this medicine be taken with meals in the morning and evening. Taking it with meals slows the rate of absorption and may reduce the incidence of orthostatic hypotension.

Precautions

This medication should not be used in certain patients. For example, individuals who have had allergic reactions to this medication or its ingredients should not be administered. It may pose risks for patients with very poor heart function or heart conduction issues and those in cardiogenic shock. Additionally, individuals with respiratory diseases such as asthma or chronic obstructive pulmonary disease, or those with a history of such diseases, should not use this medication as it may cause bronchoconstriction and exacerbate symptoms. Patients with metabolic abnormalities due to diabetes, severe liver dysfunction, pregnant women or those who may become pregnant, and breastfeeding individuals should also avoid this medication. Individuals with peripheral vascular disorders should be cautious, as this medication may impair blood circulation. Moreover, those who are genetically lactose intolerant should not take this medication. When using this medication, patients with diabetes, severe allergic reactions, or sensitivity to beta-blockers should be given cautious dosing. Elderly patients and those with low blood pressure should be particularly careful when starting treatment or increasing doses. Additionally, caution is necessary for patients with other heart conditions or those taking certain medications concurrently. Light symptoms such as dizziness, headache, and fatigue may appear with this medication, and rarely, mental symptoms like hallucinations, depression, or sleep disorders may arise. There is a risk of heart-related issues such as slowed heart rate or worsening heart function; regular check-ups are necessary. Patients should be aware of the risk of sudden drops in blood pressure or loss of consciousness. Gastrointestinal symptoms such as nausea, vomiting, and diarrhea may occur, and skin rash or itching, and in rare cases, severe allergic reactions may also happen. Long-term use of this medication requires regular monitoring of heart function, blood pressure, liver, and kidney function. Abrupt discontinuation may worsen heart disease symptoms, hence tapering off the dosage gradually is advised. When driving or operating dangerous machinery, caution is needed due to potential dizziness or drowsiness. Interactions with other medications may occur when taking this drug, so it is essential to consult with a physician or pharmacist. Extra care is particularly required when used with medications affecting blood pressure or heart rate, diabetes medications, sedatives, or antidepressants. Pregnant or breastfeeding women should not take this medication, and women who may become pregnant must confirm their status before use. Safety in children or adolescents under 18 has not been established, so use is not recommended. Elderly patients may experience increased drug concentration due to reduced liver function and may be at higher risk for stroke due to excessive blood pressure reduction, so starting with a lower dose and closely observing the condition is recommended. In the case of an overdose, symptoms like low blood pressure, decreased heart rate, weakened heart function, shock, and respiratory distress may occur, requiring immediate medical assistance. This medication may discolor due to light, so it should be stored in an opaque container away from children's reach.
