Efficacy & effects

Improvement of blood glucose control in type 2 diabetes

View approved original text

This medication is used as an adjunct to diet and exercise to improve blood glucose control in patients with type 2 diabetes. It can also be administered as monotherapy, and in cases where control is difficult with oral hypoglycemics alone, it may be combined with insulin. Additionally, if control is difficult with sulfonylureas or metformin as monotherapy, it can be combined with metformin for more effective blood glucose control. Thus, this medication is utilized to improve blood glucose control according to various therapeutic situations.

How to take

Adult dose

Administer orally once a day. Swallow without chewing with more than half a cup of water. It should be taken before breakfast or immediately before the first meal, and it is crucial not to skip meals after taking the medication. The initial dose starts at 1 mg once a day, and the dose may be increased by 1 mg at intervals of 1 to 2 weeks. Generally, there are minimal additional effects when doses exceed 4 mg; however, some patients may observe improved metabolic control at 6 mg.

Precautions

This medication should not be used in patients with insulin-dependent diabetes, diabetic ketoacidosis, or in a comatose state. It is also advisable to avoid its use in patients who have had an allergic reaction to this medication or its components, or in those with severe liver or kidney dysfunction. Pregnant women or women who might become pregnant, and breastfeeding women should also avoid using this medication. In cases of severe infections, surgeries, or severe trauma, insulin therapy is recommended, so this medication should be avoided. Patients with gastrointestinal disorders should be cautious as difficulty absorbing food may increase the risk of hypoglycemia. This medication contains lactose and should not be administered to patients with lactose intolerance or certain genetic conditions. The initial risk of hypoglycemia is high, so special observation is necessary, particularly in elderly patients or those with poor nutritional status, those skipping meals, and patients with weakened kidney or liver function. Caution should also be exercised when consuming alcohol or combining with other medications, as this may increase the risk of hypoglycemia. Patients with an allergy to Yellow No. 4 should also be administered this medication with caution. Symptoms of hypoglycemia may manifest as headaches, severe hunger, nausea, tiredness, confusion, drowsiness, and decreased consciousness. In cases of hypoglycemia, it is essential to ingest sugar to rapidly alleviate symptoms, and in severe cases, urgent treatment from medical personnel is required. Temporary visual disturbances or gastrointestinal symptoms (nausea, vomiting, abdominal pain, etc.) may occur, and abnormal liver function or blood symptoms may rarely occur. Allergic reactions including skin rashes, itching, and severe cases like respiratory distress or shock may present, and if such symptoms arise, medical personnel should be notified immediately. This medication should be taken regularly alongside dietary management and exercise, and it is crucial to regularly check blood glucose control status. To prevent hypoglycemia, patients should comprehensively understand sugar intake methods and symptoms of hypoglycemia, and it is advisable to always carry sugar for emergencies. This medication may interact with other medications, so if there are other medications being taken, informing a doctor or pharmacist is essential. Especially when combined with medications metabolized by the liver, caution is needed as it may affect blood glucose control. Pregnant or breastfeeding women should refrain from using this medication and switch to insulin treatment for safety. Safety in children under 18 years has not been confirmed, so it is preferable not to use this medication in that age group. In elderly patients, dose adjustments may be necessary due to possible increased drug concentrations from decreased kidney function. In case of an overdose, severe hypoglycemia may occur, and immediate medical attention is required. In emergencies, intravenous glucose administration should be conducted promptly to normalize blood glucose levels. This medication should be stored out of the reach of children and kept in its original container to maintain the safety and efficacy of the drug.
