Treatment of mood disorders (depression and premenstrual dysphoric disorder), obsessive-compulsive disorder, and bulimia nervosa
This drug is used to improve symptoms of depression, bulimia nervosa, obsessive-compulsive disorder, and premenstrual dysphoric disorder. Effects are typically observed within 5-6 weeks in patients with depression, alleviating persistent low mood accompanied by impairment in daily functioning and various physical and mental symptoms. In bulimia nervosa, the drug helps reduce binge eating and purging behavior. In obsessive-compulsive disorder, it reduces obsessive thoughts and behaviors, alleviating discomfort in social and work life. In premenstrual dysphoric disorder, it improves symptoms such as depression, anxiety, and physical discomfort prior to menstruation, helping to prevent interference with daily life. However, long-term use requires ongoing assessment of effectiveness. Overall, this drug is beneficial for improving mental and physical symptoms.
Adults 1. Depression 1) Initial treatment: The initial dose is fluoxetine hydrochloride 20mg orally once daily in the morning. In controlled clinical trials, doses of 20~80mg daily were administered. If no improvement is observed, a dose increase may be considered after several weeks. Doses exceeding 20mg daily should be administered in 2 divided doses (morning and noon), and the maximum dose of 80mg daily should not be exceeded. The onset of an adequate antidepressant effect may be delayed until 4 weeks or longer after starting treatment. 2) Maintenance/continuation/extended treatment: There are no definitive data on how long patients should be treated with this medicine; however, psychopharmacologists recognize that acute episodes of depression require several months or longer of drug treatment. It is not known whether the amount of antidepressant needed to relieve symptoms is the same as that needed to maintain or sustain remission. 2. Bulimia nervosa Administer 60mg of this medicine daily. The daily dose should not exceed 80mg. Measure electrolyte concentrations before starting treatment. 3. Obsessive-compulsive disorder Administer 20~60mg of this medicine daily. The daily dose should not exceed 80mg. 4. Premenstrual dysphoric disorder Administer 20mg of this medicine daily. Reassess whether continued treatment is necessary after 6 months of treatment. Patients with renal and/or hepatic impairment, patients receiving multiple medicines, elderly patients, and patients with comorbid conditions: Administer a low dose or reduce the frequency of administration. Adjust the dose appropriately according to age and symptoms. Switching from or to MAO inhibitors for the treatment of psychiatric disorders When discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this medicine, allow an interval of at least 14 days. Conversely, at least 5 weeks must elapse after discontinuing this medicine before starting an MAO inhibitor for the treatment of psychiatric disorders. Other MAO inhibitors, such as linezolid or methylene blue Patients receiving linezolid or intravenous methylene blue should not start treatment with this medicine because of the increased risk of serotonin syndrome. In patients who require more urgent treatment for a psychiatric condition, including hospitalization or other interventions, treatment should be considered. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this medicine. If no alternative to linezolid or intravenous methylene blue is available and, for a particular patient, the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome, this medicine may be stopped immediately and linezolid or intravenous methylene blue administered. Monitor the patient for symptoms of serotonin syndrome for 5 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever occurs first. Treatment with this medicine may be resumed 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by a non-intravenous route (oral tablets or local injection), or with intravenous administration of this medicine at a dose of 1mg/kg or less, is unclear. Nevertheless, clinicians should be aware of the possibility of emergency symptoms of serotonin syndrome with these uses.
This medication is used to treat psychiatric conditions such as depression and caution is especially needed in children and adolescents, as its use is not approved in these populations. This drug may increase the risk of suicidal thoughts or behaviors, especially in young adults aged 18 to 24, so caution is advised when prescribing it to this demographic. All patients taking this medication should be closely monitored for suicidal tendencies or changes in behavior during the initial treatment period and when changing dosages, and family members and caregivers should also keep a close watch on the patient. This drug should not be used in specific patients, such as those with allergies to this medication or its components, patients with severe renal impairment, and patients currently taking MAO inhibitors. Caution is required when prescribing this medication to patients with a history of seizures; treatment should be discontinued immediately if seizures occur or their frequency increases. This medication should also be used with caution in patients with a history of mania or hypomania and should be stopped if symptoms arise. Patients with heart disease or at risk of QT interval prolongation must have an ECG prior to taking this drug and their heart condition closely monitored during treatment. Patients with impaired liver or kidney function may need dosage adjustments or cautious use. Diabetic patients should monitor their blood sugar levels closely, as changes in blood sugar control may occur during treatment. Rarely, this medication can cause dilation of pupils, necessitating caution in patients at risk for increased intraocular pressure or acute glaucoma. Common side effects of this drug include headache, nausea, insomnia, fatigue, and diarrhea, while allergic reactions or severe skin reactions may occur infrequently. Withdrawal symptoms such as dizziness, sensory disturbances, sleep disorders, anxiety, nausea, trembling can occur upon discontinuation, so dosing should be tapered off gradually. This drug may interact with other medications, so patients should inform their doctor or pharmacist of any other medications they are taking. Particularly, the concurrent use with MAO inhibitors can lead to life-threatening serotonin syndrome, and such combinations should be avoided. During pregnancy, the risks to the fetus and the benefits of this medication should be carefully weighed, and caution is needed during breastfeeding as the drug can be excreted in breast milk. The safety of this drug in children and adolescents has not been established; thus, avoidance of use is recommended if possible. Elderly patients might experience adverse effects from this medication, thus requiring careful monitoring. Overdose can lead to symptoms such as nausea, vomiting, seizures, heart arrhythmias, and coma, and immediate medical attention must be sought. This drug should be stored out of the reach of children and should not be transferred to other containers. Patients should be careful while driving or operating dangerous machinery during treatment and are advised to avoid alcohol consumption.
