Treatment of psychiatric disorders (depression, obsessive-compulsive disorder), bulimia nervosa, and premenstrual dysphoric disorder
This drug helps improve symptoms of depression patients within approximately 5-6 weeks. It also reduces binge eating and laxative behaviors in patients with bulimia nervosa and significantly alleviates obsessive symptoms in obsessive-compulsive disorders. It is also used to relieve emotional instability and physical symptoms due to premenstrual dysphoric disorder. However, regular evaluation of effectiveness is necessary for long-term use in each condition. Overall, it is used for the treatment of depression, bulimia nervosa, obsessive-compulsive disorder, and premenstrual dysphoric disorder.
Depression Initial treatment: The initial dose is 20mg of fluoxetine hydrochloride orally once daily in the morning. In controlled clinical trials, doses of 20~80mg daily were administered. If no improvement is observed, an increase in the dose may be considered after several weeks. Doses exceeding 20mg daily should be administered twice daily (morning and noon), and the maximum dose of 80mg daily should not be exceeded. The onset of an adequate antidepressant effect may be delayed until 4 weeks of treatment or longer. Maintenance/continuation/extended treatment: There are no definitive data on how long patients should be treated with this drug, but psychopharmacologists agree that acute episodes of depression require drug treatment for several months or longer. It is not known whether the amount of antidepressant needed to relieve symptoms is the same as the amount needed to maintain or continue stabilization. Bulimia nervosa Administer 60mg of this drug daily. The daily dose should not exceed 80mg. Measure electrolyte levels before starting treatment. Obsessive-compulsive disorder Administer 20~60mg of this drug daily. The daily dose should not exceed 80mg. Premenstrual dysphoric disorder Administer 20mg of this drug daily. After 6 months of treatment, reassess whether continued treatment is necessary. Patients with renal and/or hepatic impairment, patients receiving multiple drugs, older adults, and patients with comorbid diseases: Administer a lower dose or reduce the frequency of administration. Adjust the dose appropriately according to age and symptoms.
This medication may increase the risk of suicidal thoughts and behaviors in children, adolescents, and young adults with major depressive disorder or psychiatric conditions, so careful judgment is required when administering it to this age group. While taking this medication, there may be abnormal behavioral changes such as suicidal thoughts, aggression, or anger, and patients and their families should carefully monitor for these symptoms. Patients who are allergic to this medication or its ingredients, those with severe renal impairment, or those currently taking certain types of medications (MAO inhibitors) should not use this medication. Patients with a history of seizures should be especially cautious when using this medication, and treatment should be immediately discontinued if seizures occur or increase in frequency. Patients with depression may temporarily experience an increased risk of suicide at the beginning of treatment or when the dose is changed, necessitating careful monitoring. Patients with a history of mania or hypomania should use this medication with caution, and if manic symptoms occur, treatment should be discontinued. Patients with cardiovascular conditions or a risk of QT interval prolongation should have an ECG before taking this medication, and treatment should be immediately stopped if abnormal signs appear. Patients with impaired liver or kidney function should use this medication cautiously with dose adjustments. Diabetic patients may experience changes in blood sugar while taking this medication and should monitor their blood sugar levels closely. Patients with increased intraocular pressure or at risk for glaucoma should use this medication cautiously due to side effects like pupil dilation. Common side effects of this medication include headache, nausea, insomnia, fatigue, and diarrhea, and rare but severe allergic or serious skin reactions may occur. Sudden discontinuation of this medication may lead to withdrawal symptoms such as dizziness, sensory changes, anxiety, nausea, and tremors; therefore, it should be gradually tapered under a doctor's guidance. There may be interactions with other medications, so patients should discuss all medications they are taking with their physician. During pregnancy, the risks to the fetus and the benefits of this medication should be carefully weighed, and caution is needed during breastfeeding as the medication may be passed through breast milk. The safety of this medication in children and adolescents has not been established, and its use is not recommended in these groups. Elderly patients should be cautious as the risk of side effects may vary based on their physical condition. In cases of overdose, severe cardiac issues or central nervous system symptoms may occur, requiring immediate medical action. This medication may impact judgment, thinking, and motor skills, so caution is advised when driving or operating dangerous machinery.
