Efficacy & effects

Improvement of blood sugar control in type 2 diabetes

  • Blood sugar control abnormalities in patients with type 2 diabetes
View approved original text

This medication assists in dietary and exercise therapy to enhance blood sugar control in patients with type 2 diabetes. It can be administered alone, and in cases where blood sugar control is insufficient, it may be used in combination with oral hypoglycemic agents or insulin. Specifically, it is used in combination with metformin when the effects of sulfonylureas or metformin monotherapy are inadequate. This multifaceted treatment effectively manages blood sugar to aid in diabetes management.

How to take

Adult dose

Administered orally once daily. The initial dose is 1 mg of glimepiride, and it is increased by 1 mg at 1 to 2 week intervals. Generally, there is little additional effect when the dose exceeds 4 mg; however, if needed, it may be increased to up to 6 mg (or 8 mg). In combination therapy: - When used in combination with insulin, take 8 mg orally with the first meal of the day, and adjust insulin dosage based on fasting blood sugar.

Precautions

This medication should not be administered to patients with insulin-dependent diabetes, diabetic ketoacidosis, or coma. Patients who have had allergic reactions to this medication or its components, or to sulfonylureas should also refrain from use. This medication is not used in patients with severe liver or kidney dysfunction, and in such cases, transitioning to insulin therapy is recommended. Pregnant or potentially pregnant women, as well as nursing mothers, should not take this medicine. Patients with severe infections, or those undergoing surgery or with severe injuries may require insulin therapy, so caution is needed when using this medication. In cases where food absorption is impaired due to diarrhea or vomiting, there is a risk of hypoglycemia, so caution must be exercised. During the first week of taking this medication, the risk of hypoglycemia is high, so blood sugar levels should be closely monitored. Special care should be taken in elderly patients, those with poor nutritional status or skipping meals, or those with an imbalance of exercise and carbohydrate intake. Patients who frequently consume alcohol or have impaired kidney and liver function may also have an increased risk of hypoglycemia. Additionally, having other endocrine disorders or taking certain medications could also increase the risk of hypoglycemia. It is recommended to consult with a doctor or pharmacist regarding dosage or administration when there are such risk factors. The most common side effect of this medication is hypoglycemia, which may present symptoms such as headache, severe hunger, nausea, fatigue, anxiety, confusion, drowsiness, or decreased consciousness. Hypoglycemia should be treated immediately by taking sugar, and in severe cases, emergency treatment may be required. Furthermore, initial blood sugar changes may lead to blurred vision, gastrointestinal symptoms, liver function abnormalities, blood disorders, or allergic reactions, so any unusual symptoms should be reported to medical personnel immediately. Rarely, serious allergic reactions or liver failure may occur, necessitating caution. When taking this medication, regular meals, exercise, and weight management are important, and blood sugar control should be continuously monitored. Patients and their families should be thoroughly informed about the symptoms and risk factors of hypoglycemia, and preparations should be made to consume sugar immediately upon hypoglycemic episodes. Extra careful monitoring is needed, especially at the beginning of treatment or when changing dosages. Stressful situations or surgery and infections may complicate blood sugar management, sometimes necessitating a switch to insulin therapy. This medication has many interactions with other drugs, so it's essential to inform a doctor or pharmacist if taking other medications or stopping any medications. Special caution is needed when taken alongside medications that lower or raise blood sugar levels, as this can affect blood sugar control. Additionally, since this medication is metabolized in the liver, caution is advised when used with drugs that affect liver metabolism. Pregnant or nursing mothers should not take this medication, as it may harm the fetus or newborn. If planning a pregnancy, consultation with a doctor to switch to insulin therapy is advisable. This medication has not been proven safe and effective for children under 18 years of age, so it should not be used in this population. In elderly patients, renal function may be reduced, requiring careful adjustment of dosage. In cases of overdose, severe hypoglycemia may occur, so immediate emergency treatment must be sought at a medical facility, and treatments such as inducing vomiting, blocking drug absorption, or intravenous glucose may be necessary. This medication should be stored out of reach of children and kept in its original container to prevent misuse and deterioration of quality.
