Improvement of chronic arterial occlusion (limb ulcer, pain at rest), improvement of skin ulcers (progressive systemic sclerosis, systemic lupus erythematosus), recovery effects in vibrational disease (peripheral blood flow disorders, neurological and motor function disorders)
This medication helps improve limb ulcers and pain at rest in vascular diseases such as chronic arterial occlusion. It is also used to treat skin ulcers associated with progressive systemic sclerosis and systemic lupus erythematosus. For patients with vibrational disease, it aids in alleviation of subjective symptoms due to peripheral blood circulation disorders and in recovery of peripheral circulation and neurological and motor function. It contributes to the maintenance of ductus arteriosus patency in ductus arteriosus dependent congenital heart diseases. It is effective in treating skin ulcers in diabetic patients and supports blood flow maintenance after vascular reconstruction surgery. Additionally, it is used to enhance imaging capabilities during superior mesenteric artery portography to help overall blood circulation improvement.
1. For chronic arterial occlusion (Buerger's disease, occlusive arterial sclerosis), progressive systemic sclerosis, systemic lupus erythematosus, vibrational disease, diabetic skin ulcers: Typically, 1-2 milliliters (5-10 micrograms of Alprostadil) once daily for adults, administered as is or mixed with fluids for slow intravenous or drip infusion. Adjust the dose according to symptoms. 2. For ductus arteriosus dependent congenital heart disease: Initially infused mixed in fluids at 5ng/kg/min after Alprostadil initiation and then adjusted based on symptoms to the minimum effective dose. 3. For improving imaging capabilities during superior mesenteric artery portography: For typically adults, dilute 1 milliliter (5 micrograms of Alprostadil) in 10 milliliters of saline and administer via a catheter into the superior mesenteric artery over 3 to 5 seconds, 30 seconds before the contrast agent injection. 4. For maintaining blood flow after vascular reconstruction: Typically for adults, infuse 2 milliliters (10 micrograms of Alprostadil) mixed with fluids once daily via drip infusion.
This drug can cause apnea when administered to newborns with ductus arteriosus dependent congenital heart disease, so it must be used in a facility equipped with respiratory management equipment. When cutting the container of the ampule injection, glass fragments may be mixed in, so caution must be exercised, particularly when administering to children or the elderly. It should not be administered to patients with severe heart failure or hemorrhage, pregnant women or women of childbearing potential, patients with hypersensitivity to this drug or its components, and patients allergic to soybean oil, soybeans, or peanuts. Caution should be exercised when administering to patients with a history of heart failure, glaucoma, gastric ulcers, interstitial pneumonia, renal failure, bleeding tendencies, patients on anticoagulants or platelet function inhibitors, and patients with moderate esophageal varices on specific portography, and patients with abnormal lipid metabolism should be administered with care. During administration, adverse reactions such as blood pressure changes, liver function abnormalities, redness at the injection site, headaches, diarrhea, nausea, fever, etc., may occur, so the patient's condition should be closely monitored. Particularly in newborns, apnea, hyponatremia, and fever may occur frequently, so careful observation is necessary. In severe cases, serious adverse reactions such as shock, anaphylaxis, loss of consciousness, heart failure, pulmonary edema, interstitial pneumonia, myocardial infarction, cerebral hemorrhage, and blood cell reduction may occur, so if any abnormal symptoms appear, the administration should be stopped immediately, and appropriate treatment should be provided. Elderly patients should have their dose adjusted as physiological functions are reduced, so care should be taken during administration. This drug can increase the risk of bleeding when used in conjunction with anticoagulants, platelet function inhibitors, and thrombolytics, so patients taking these medications should be sure to inform their doctor beforehand. This drug should not be administered to pregnant women or women of childbearing potential, and its safety for children other than ductus arteriosus dependent congenital heart disease has not been established. When administering, do not mix with other medications, especially avoid direct mixing with contrast agents, and the catheter should be flushed with saline after administration. Additionally, this drug should be used within 24 hours after mixing with fluids, and any unused portions should be discarded, and frozen medications should not be used. When using polyvinyl chloride fluid sets, di(2-ethylhexyl)phthalate (DEHP), a plasticizer, may leach out, so it is advisable to avoid using these fluid sets in newborns.
Not everyone experiences these, and they can vary from person to person.
This drug should be stored in a sealed container to prevent exposure to air or moisture. Also, to maintain the drug's efficacy, it should be stored in a cool place away from light and should not freeze.
