Treatment of schizophrenia and bipolar disorder (including manic and depressive episodes); prevention of recurrence
This medication is used for the treatment of schizophrenia and bipolar disorder. It helps alleviate the symptoms of schizophrenia patients and improve their quality of life. Additionally, it is effective for the treatment of mania and mixed episodes associated with bipolar disorder type I, and it aids in the prevention of relapse in patients responding to olanzapine for manic episodes. Furthermore, it is used for the acute treatment of depressive episodes in bipolar disorder type I, improving symptoms. Thus, this medication stabilizes various symptoms and contributes to the alleviation of patients' symptoms.
The therapeutic dose of this medicine should be adjusted according to the patient's clinical condition. Increases above the usual recommended daily dose of 10 mg are recommended only after an appropriate clinical evaluation. 1. Adults 1) Schizophrenia (1) The recommended initial dose is 5∼10 mg of olanzapine once daily. (2) Maintenance treatment: Although it is difficult to give a definitive answer as to how long treatment with this medicine should continue, its efficacy was demonstrated in a placebo-controlled study in patients with schizophrenia whose symptoms had stabilized after approximately 8 weeks of treatment with this medicine and who received maintenance treatment at a dose of 10∼20 mg daily for 8 months. The need for maintenance treatment at an appropriate dose should be assessed periodically. 2) Bipolar disorder (1) Treatment of manic and mixed episodes associated with bipolar I disorder The initial dose is 15 mg of this medicine once daily as monotherapy, or 10 mg once daily as adjunctive therapy with lithium or valproate. (2) Prevention of recurrence in bipolar disorder The recommended initial dose is 10 mg once daily. In patients who have received this medicine to treat mania, continue the same dose to prevent recurrence. If a new manic, mixed, or depressive episode occurs, treatment with this medicine should continue, together with adjunctive therapy to treat mood symptoms (optimizing the required dose) according to clinical indications. 3) For schizophrenia and the treatment of manic and mixed episodes associated with bipolar I disorder, and for prevention of recurrence in bipolar disorder, the daily dose may be adjusted within the range of 5 mg to 20 mg per day based on clinical status. Increases above the recommended initial dose are recommended only after an appropriate clinical evaluation and should generally be made at intervals of 24 hours or more. If treatment with this medicine is discontinued, the dose should be reduced gradually and cautiously. 4) Acute treatment of depressive episodes associated with bipolar I disorder Start treatment with 5mg of olanzapine once daily, then increase to 10mg once daily. Administer this medicine at bedtime and adjust the dose appropriately according to age and symptoms; do not exceed a daily dose of 20mg. Since food does not affect drug absorption, this medicine may be taken with or without food. 2. Dosing in special populations 1) Older adults Starting at a dose of 5 mg or less per day may be considered when there is sufficient evidence based on clinical factors. 2) Patients with renal or hepatic impairment A low initial dose (5 mg) should be considered. In cases of moderate hepatic impairment (cirrhosis, Child-Pugh class A or B), the initial dose is 5 mg, and the dose should be increased cautiously. 3) Patients with multiple factors that delay the metabolism of this medicine (female sex, older age, and nonsmoking status) may also start at a low dose.
This medication can cause a serious side effect known as neuroleptic malignant syndrome, which can be life-threatening. If you experience excessive fever, muscle stiffness, or changes in mental state, you should immediately discontinue use and seek medical attention. It is not recommended for the treatment of psychosis or behavioral disorders related to dementia, as there may be an increased risk of mortality and cerebrovascular accidents, particularly in elderly patients. Prolonged use may lead to tardive dyskinesia, a movement disorder, and if symptoms occur, dosage adjustment or discontinuation should be considered. This medication may elevate blood glucose levels and worsen diabetes, so diabetic patients or those with risk factors should monitor their blood glucose and weight regularly. Drowsiness or dizziness may occur during treatment, and caution is advised when driving or operating hazardous machinery. Patients with poor liver function or liver disease should have regular liver function tests, and if hepatitis occurs, treatment should be discontinued. This medication can decrease white blood cell counts, so if symptoms of leukopenia appear, consult a doctor immediately. Pregnant women or nursing mothers, or those planning to become pregnant, should consult a doctor before taking this medication, as it may affect the fetus or newborn. Abrupt discontinuation of this medication may result in symptoms such as insomnia, anxiety, or nausea; therefore, the dosage should be gradually reduced according to a doctor's instructions. If you are taking other medications, especially those that may affect enzymes that break down this medication in the liver, or if you use it in conjunction with epinephrine, you must inform your doctor or pharmacist. The safety in children or adolescents has not been sufficiently evaluated, so special attention is required for those under 18 years of age. In the case of an overdose, symptoms such as decreased consciousness, seizures, or heart problems may occur, so seek medical care immediately.
