Treatment of idiopathic Parkinson's disease and moderate to severe primary restless legs syndrome
This medication is used to treat idiopathic Parkinson's disease. It can be used alone or in combination with levodopa, which stabilizes the effects of the medication and reduces the on-off phenomenon that may occur as a side effect. Additionally, it can lower the daily dosage of levodopa, thereby reducing the treatment burden. This medication is also effective for the treatment of moderate to severe primary restless legs syndrome. Therefore, this drug is used for the treatment of Parkinson's disease and restless legs syndrome.
(1) Idiopathic Parkinson's disease It is preferable to take this medicine 3 times daily with food to improve gastrointestinal tolerability. Initial therapy: Take 0.25mg 3 times daily. The treatment regimen for the first 4 weeks is as follows: Week 1: Single dose 0.25mg, daily dose 0.75mg Week 2: Single dose 0.5mg, daily dose 1.5mg Week 3: Single dose 0.75mg, daily dose 2.25mg Week 4: Single dose 1.0mg, daily dose 3.0mg Maintenance therapy: After initial therapy, the dose may be increased at 1-week intervals up to 3mg daily. A therapeutic effect is seen at doses of 3-9mg daily. If symptoms are not adequately controlled or symptom control is not maintained, the ropinirole dose may be increased until a clear therapeutic effect is achieved; however, it should not exceed 24mg daily. ① When administered as adjunctive therapy to levodopa, the levodopa dose may be gradually reduced to approximately 20% of the total dose. ② When switching from another dopamine agonist to ropinirole, follow the manufacturer's guidelines for discontinuation before starting ropinirole. ③ As with other dopamine agonists, ropinirole should be discontinued gradually by reducing the number of daily doses over the course of 1 week. (See section ‘4. General precautions’ under Precautions for use.) (2) Primary restless legs syndrome Ropinirole should be taken once daily before bedtime, 1~3 hours before going to bed. Initial therapy (week 1): The recommended initial dose is 0.25mg once daily for 2 days, taken 1~3 hours before bedtime. If this dose is well tolerated, it may be increased to 0.5mg once daily starting on day 3 of treatment through the last day of the first week of treatment (day 7 of treatment). Maintenance therapy (from week 2): After initial therapy, the daily dose may be increased according to the following schedule until an optimal therapeutic response is achieved. Week 2: Single dose 1mg Week 3: Single dose 1.5mg Week 4: Single dose 2mg Week 5: Single dose 2.5mg Week 6: Single dose 3mg Week 7: Single dose 4mg The safety and efficacy of doses greater than 4mg/day have not been established in patients with restless legs syndrome. As with other dopamine agonists, treatment with this medicine should be discontinued gradually by reducing the daily dose. (See section ‘4. General precautions’ under Precautions for use.) If treatment with this medicine has been interrupted significantly, it is recommended that treatment be restarted using the dose titration described above. 2. Patients with renal impairment: Since ropinirole clearance is unchanged in patients with mild or moderate renal impairment (creatinine clearance 30-50mL /min), no dose adjustment is necessary. Use of this medicine has not been studied in patients with severe renal impairment (creatinine clearance <30mL/min); therefore, ropinirole is not recommended in these patients. 3. Patients with hepatic impairment: Use of this medicine has not been studied in patients with hepatic impairment; therefore, ropinirole is not recommended in these patients. 4. Use in older adults: Ropinirole clearance is reduced in patients aged 65 years or older, but the dose for older patients may be adjusted according to the patient's clinical response in the usual manner.
Administration to children: Parkinson's disease does not occur in children. Therefore, no studies on the use of ropinirole in this population have been reported, and it should not be administered to children.
This medication should be used with caution in patients with severe cardiovascular diseases. When taken with antihypertensive or antiarrhythmic medications, side effects such as hypotension, bradycardia, and arrhythmia may occur, thus requiring caution. Patients with significant mental illnesses should only use this medication when the benefits of treatment are deemed greater than the risks. Sudden extreme drowsiness or abrupt sleepiness may occur during the day while taking this medication, and if such symptoms appear, patients should avoid hazardous activities such as driving or operating machinery. Patients with hypersensitivity to this medication, pregnant or breastfeeding women, and those with specific genetic lactose intolerance should not take this medication. Various side effects may occur, including nausea, vomiting, drowsiness, dizziness, hypotension, peripheral edema, etc., so if any unusual symptoms occur, a doctor should be consulted. Children under 18 years old are not recommended to use this medication as its safety and efficacy have not been established in this age group. In the case of overdose, symptoms such as nausea, dizziness, hallucinations, dyskinesia, and severe drowsiness may occur; hence the dosage must be strictly adhered to. This medication should be kept out of reach of children, stored in a cool, dry place away from direct sunlight, and at or below 25 degrees Celsius. During treatment, impulse control disorders or psychiatric symptoms (e.g., hallucinations, delirium) may occur; therefore, regular observation is necessary. Abruptly stopping the medication can result in withdrawal symptoms such as insomnia, depression, and fatigue, so it is important to taper gradually. Smoking or taking certain medications can affect the blood concentration of this medication, so changes in condition should be reported to healthcare professionals during treatment.
Not everyone experiences these, and they can vary from person to person.
