Adult dosage
For clindamycin phosphate in severe infections, administer 600 to 1200 mg (units) per day divided into 2 to 4 intravenous or intramuscular injections. For severe infections, administer 1200 to 2700 mg (units) per day divided into 2 to 4 injections. In emergencies, the daily dose may be increased up to 4.8 g (units) and administered by intravenous injection. The injection should not be injected directly into the vein without dilution, and it should be given over at least 10 to 60 minutes. Injections should be administered to maintain clindamycin blood levels above a specified threshold by following the rapid infusion rates indicated in the table below during the first injection and then applying the maintenance infusion rates. Blood concentration maintenance line Rapid infusion rate Maintenance infusion rate Above 4 μg/mL 10 mg/min for 30 minutes 0.75 mg/min Above 5 μg/mL 15 mg/min for 30 minutes 1.00 mg/min Above 6 μg/mL 20 mg/min for 30 minutes 1.25 mg/min However, it is not recommended to administer a single dose of over 600 mg intramuscularly. For children (1 month and older): administer 15 to 25 mg (units) per kg body weight divided into 3 to 4 intravenous or intramuscular injections for severe infections. For severe infections, administer 25 to 40 mg per kg body weight divided into 3 to 4 injections. For newborns (under 1 month): administer 15 to 20 mg (units) per kg body weight divided into 3 to 4 injections. For preterm infants, lower doses may be sufficient. If symptoms improve, the method of administration may change to oral therapy (capsules, syrup) at the physician's discretion. For beta-hemolytic streptococcus infections, it is desirable to continue administration for at least 10 days. If diarrhea occurs during treatment, discontinue the administration (refer to warnings). Adjust according to age and symptoms appropriately. Dilution method and infusion rate Clindamycin phosphate must be diluted before intravenous administration. For drip IV administration, the dilution concentration of clindamycin should not exceed 18 mg per mL, and the infusion rate should not exceed 30 mg per minute. The dilution ratio and infusion rate are as follows. Dosage Dilution for infusion Infusion time 300 mg 50 mL 10 minutes 600 mg 50 mL 20 minutes 900 mg 100 mL 30 minutes 1200 mg 100 mL 40 minutes However, it is not recommended to administer a single dose of over 1200 mg in one hour. Before administration, visually inspect the solution for particles and the discoloration of the container. Dilution and compatibility precautions The injection is diluted with solutions containing common concentrations of sodium chloride, dextrose, calcium, potassium, or vitamin B complex, and it has been confirmed through physical or biological stability studies that no inactivation or compatibility issues arise when left at room temperature for 24 hours. It is permissible to combine it with antibiotic preparations such as cephalothin, kanamycin, gentamicin, penicillin, carbenicillin, amikacin sulfate, aztreonam, cefamandole nafate, cefazolin sodium, cefotaxime sodium, cefotetan sodium, amikacin sulfate, piperacillin, and tobramycin. It is physically incompatible with ampicillin, diphenylhydantoin, barbiturates, aminophylline, calcium gluconate, magnesium sulfate, ceftriaxone sodium, phenytoin sodium, hydrochloride idarubicin, and hydrochloride ranitidine. The duration of compatibility and stability of mixtures depends on the concentration and other conditions. (Injection solution) (Gel preparation) For adults: apply thinly to the affected area twice a day. Clean the affected area before applying medication.
Children's dosage
For children (1 month and older) with severe infections, administer 15 to 25 mg (units) per kg body weight divided into 3 to 4 intravenous or intramuscular injections per day. For severe infections, administer 25 to 40 mg per kg body weight divided into 3 to 4 injections per day.

Elderly or weakened individuals, patients with liver or kidney dysfunction, atopic constitution or asthma, and those with myasthenia gravis should use this medication cautiously. Muscle injection may cause pain or numbness at the injection site, and IV injection may lead to thrombophlebitis. Gastrointestinal reactions such as diarrhea, abdominal pain, nausea, and vomiting may occur, and in severe cases, pseudomembranous colitis may develop. Allergic reactions may manifest as systemic flushing, hives, and difficulty breathing, requiring immediate discontinuation of treatment and emergency intervention. Skin reactions such as rashes, swelling, and itching can occur, and serious skin disorders may arise rarely, thus caution is necessary. Liver function impairment or kidney function impairment may occur, so regular testing is necessary during prolonged use. Musculoskeletal pain or abnormal blood symptoms have also been reported, so consult a physician if symptoms arise. This medication may interact with some drugs, so consult healthcare professionals if you are taking other medications. It is advisable not to use it in pregnant or breastfeeding women, and safety in newborns or children has not been established, so it should be administered cautiously. In the case of overdose, there is no specific antidote, and appropriate emergency management should be based on symptoms. This medication should be used as soon as possible after administration, and caution is needed as pain or spasms may occur during muscle injection. IV injections should avoid rapid administration, and the effects on driving or operating machinery are not clear, so caution is required. This medication should only be used for the minimal necessary duration to prevent the emergence of resistant bacteria, and if diarrhea or abdominal pain worsens during treatment, it should be discontinued and medical advice sought.
