Treatment and prevention of recurrence of candidiasis and cryptococcosis; treatment of cutaneous fungal infections; prevention of fungal infections
This medication is used for the treatment of various fungal infections. It is effective in treating acute or recurrent vaginal candidiasis and is used for treating candidiasis of the oral cavity, esophagus, lungs, and mucous membranes in both immunocompetent and immunocompromised patients. It is also used for systemic candidemia, disseminated candidiasis, and various invasive fungal infections. This medication is suitable for treating cryptococcal meningitis and cryptococcosis in other parts of the body, as well as for maintenance therapy to prevent recurrence in patients with AIDS. It is used to prevent fungal infections that may occur in immunosuppressed states and is also employed for treating dermatophyte infections such as onychomycosis, athlete's foot, and tinea corporis. Therefore, this medication provides effective treatment and prevention for various fungal infections.
The route of administration is determined according to the patient's clinical condition. Oral preparations are rapidly and almost completely absorbed; therefore, the doses of oral preparations (capsules, dry syrup) and injectable preparations are the same. Vulvovaginal candidiasis: Administer fluconazole 150 mg orally as a single dose. Oropharyngeal candidiasis: The usual dose of this medicine is 50 mg once daily for 7-14 days; treatment may be extended at the physician's discretion. Chronic atrophic oral candidiasis (oral pain due to dentures): The usual dose of this medicine is 50 mg once daily for 14 days; local disinfection of the dentures should also be performed. Other mucosal candidiasis (esophageal candidiasis, noninvasive bronchopulmonary infection, candiduria, and mucocutaneous candidiasis, excluding vulvovaginal candidiasis): The usual dose of this medicine is 50 mg once daily for 14-30 days. For refractory mucosal candidiasis, the dose may be increased to 100 mg once daily. Systemic candidiasis, including candidemia, disseminated candidiasis, and other invasive Candida infections (peritoneal, endocardial, pulmonary, urinary tract, etc.): The usual dose of this medicine is 400 mg once on the first day, followed by 200 mg once daily from the next day; the dose may be increased to 400 mg once daily according to the patient's response. The duration of treatment is determined according to clinical response and medical judgment. Cryptococcal meningitis and cryptococcosis at other sites: The usual dose of this medicine is 400 mg once on the first day, followed by 200-400 mg once daily from the next day. The duration of treatment is determined by clinical and mycological response, but treatment should generally continue for at least 6 ~ 8 weeks. To prevent recurrence of cryptococcal meningitis in patients with AIDS, administer 200 mg of this medicine once daily indefinitely after the patient has completed primary treatment. Prevention of fungal infections in immunocompromised patients at risk due to neutropenia caused by cytotoxic chemotherapy, radiation therapy, or bone marrow transplantation: Administer 50-400 mg of this medicine once daily, depending on the patient's risk of fungal infection. A dose of 400 mg once daily is recommended for patients at very high risk of systemic fungal infection, such as bone marrow transplant recipients with severe neutropenia or an expected prolonged period of neutropenia. In these patients, administer 400 mg beginning 5-6 days before the expected onset of neutropenia and continue 400 mg for 1 week after the neutrophil count has increased to 1,000 cells/mm3 or more. For onychomycosis of the fingernails or toenails, administer 150 mg once weekly until the nails have regrown (until new, uninfected nails have grown). Fingernails generally take 3-6 months to regrow and toenails 6-12 months; the growth rate may vary by individual and age. Occasionally, nails may remain damaged after successful treatment with long-term administration. Dermatophyte infections including tinea pedis (athlete's foot), tinea corporis, tinea cruris, and cutaneous candidiasis: The usual dose of this medicine is 50 mg once daily or 150 mg once weekly for 2-4 weeks. However, treatment of tinea pedis may be required for 6 weeks depending on the course of the disease. For tinea versicolor, administer 50 mg once daily for 2-4 weeks. Elderly patients: If renal function is normal, the usual adult dose is administered. In patients with renal impairment (creatinine clearance 50 mL/min or less), reduce the dose or extend the dosing interval. Patients with renal impairment: This medicine is excreted mainly in the urine unchanged. No dose adjustment is necessary for a single dose. However, when multiple doses are required, administer the usual dose only on the first day and adjust the dose from the next day as follows. Creatinine clearance (mL/min) > 50: 100% of the usual dose Creatinine clearance (mL/min) ≤50 (not receiving dialysis): 50% of the usual dose Patients on hemodialysis: 100% of the usual dose after each hemodialysis session Patients receiving hemodialysis should receive 100% of the usual dose after each hemodialysis session. On days without dialysis, administer a reduced dose according to creatinine clearance.
Pediatric doses based on adult doses are generally determined according to the following guidelines and should not exceed 400 mg. Adult dose 100 mg: Pediatric dose 3 mg/kg Adult dose 200 mg: Pediatric dose 6 mg/kg Adult dose 400 mg: Pediatric dose 12 mg/kg The safety and efficacy in infants younger than 6 months have not been established. Oropharyngeal candidiasis: Administer 6 mg/kg of this drug on the first day, followed by 3 mg/kg once daily from the next day. Treatment should be continued for at least 2 weeks to prevent recurrence. Esophageal candidiasis: Administer 6 mg/kg of this drug on the first day, followed by 3 mg/kg once daily from the next day. The dose may be increased to 12 mg/kg once daily depending on the patient's response. Treat for at least 3 weeks and for at least 2 weeks after symptoms resolve. Systemic candidiasis, including candidemia, disseminated candidiasis, and other invasive Candida infections (peritoneal, endocardial, pulmonary, urinary tract, etc.): Administer 6-12 mg/kg once daily. The duration of treatment should be determined according to clinical response and medical judgment. Cryptococcal meningitis: Administer 12 mg/kg of this drug on the first day, followed by 6 mg/kg once daily from the next day. The dose may be increased to 12 mg/kg once daily depending on the patient's response. Continue treatment for 10-12 weeks after cerebrospinal fluid cultures become negative. To prevent recurrence of cryptococcal meningitis in patients with AIDS, administer 6 mg/kg once daily after the patient has completed all initial treatment. Pediatric patients with renal impairment should receive longer dosing intervals and lower doses according to the same criteria as adult patients with renal impairment. When administering doses below 10 mg, administer the syrup orally according to the child's clinical condition, or administer the injection intravenously, to ensure accurate dosing.
This medication should not be administered to certain patients. For example, patients who have had allergic reactions to this drug or similar drugs, pregnant women or women who may become pregnant, and nursing mothers should not use this medication. Additionally, serious side effects can occur when used with certain drugs, so patients taking concomitant medications that are contraindicated with this drug should also avoid its use. Because this medication can affect liver and kidney function, patients with liver or kidney disease require careful assessment and monitoring before administration. Special caution is needed for patients with heart diseases or those at high risk of arrhythmias due to electrolyte imbalances. Although this drug is generally well tolerated, abnormal reactions can rarely occur in multiple areas such as blood, liver, kidneys, skin, and nervous system, so if any abnormal symptoms occur, immediate consultation with medical personnel is advised. In particular, this drug can prolong the QT interval related to heartbeats, which may increase the risk of serious arrhythmias, so particular care should be taken for patients with heart disease or those taking specific drugs. It should not be used by pregnant women as it may harm the fetus, and women of childbearing age must use effective contraception during treatment and for a certain period after administration. Nursing mothers should also avoid use as this drug is excreted in breast milk. Caution is required during driving or operation of machinery since dizziness or seizures may occur while taking this medication. Overdosage may lead to hallucinations or abnormal behavior, which may require immediate treatment and possibly blood dialysis. Finally, this drug interacts with multiple other medications, so dosage adjustments or monitoring may be necessary according to concomitant drugs; all current medications should be reported to medical personnel for consultation.
