Prevention and treatment of transplant rejection and GVHD; treatment of immune disorders (psoriasis, etc.) and aplastic anemia
This medication is used to prevent rejection reactions after the transplantation of various organs such as the kidneys, liver, lungs, and heart in patients who have undergone organ transplantation. It is also used to treat rejection reactions in patients already on immunosuppressive therapy and to treat graft-versus-host disease (GVHD) following bone marrow transplantation. This drug is helpful in severe psoriasis cases where other treatments have failed or are inappropriate. For nephrotic syndrome, it is used to induce and maintain symptom relief in steroid-dependent or resistant patients who have failed treatment with cell proliferation inhibitors, suitable for patients with kidney function above a certain level. Additionally, it is useful in the treatment of severe aplastic anemia and severe rheumatoid arthritis that do not respond to standard therapy. It effectively improves and treats symptoms in various immune-related diseases.
1. Organ transplantation: The initial dose is 10 ∼ 15 mg/kg of body weight per day in 2 divided doses, starting 12 hours before surgery. For 1 ∼ 2 weeks after surgery, administer the same dose as the initial dose for maintenance, then gradually reduce the dose according to blood concentrations until reaching 2 ∼ 6 mg/kg of body weight per day in 2 divided doses. 2. Bone marrow transplantation: Administer 12.5 ∼ 15 mg/kg of body weight per day in 2 divided doses on the day before surgery. For maintenance, administer approximately 12.5 mg/kg of body weight per day for 3 ∼ 6 months, until 1 year after transplantation. 3. Psoriasis: The initial dose is 2.5 mg/kg of body weight in 2 divided doses per day. If there is no improvement after 4 weeks, the dose may be increased by 0.5 ~ 1 mg/kg of body weight per day each month, up to 5 mg/kg of body weight per day. 4. Nephrotic syndrome: In adults, administer 5 mg/kg per day in 2 divided doses. 5. Rheumatoid arthritis: Administer 3 mg/kg of body weight per day orally in 2 divided doses during the first 6 weeks of treatment. If symptom improvement is insufficient after 6 weeks, the dose may be increased to 5 mg/kg of body weight per day. 6. Aplastic anemia: Usually, administer 6 mg/kg of body weight per day in 2 divided doses, and reduce the dose to 4 mg/kg of body weight per day according to blood concentrations. 7. The total daily dose of this drug should always be administered in 2 divided doses.
4. Nephrotic syndrome: In children, administer 6 mg/kg per day in 2 divided doses. However, in patients with renal function below the specified limit, the initial dose should not exceed 2.5 mg/kg per day.
As this medication is an immunosuppressant, it should not be administered to certain patients. It should not be given to patients with allergies or hypersensitivity to this medication or its components, patients with kidney disease who have uncontrolled infections or malignant tumors, psoriasis patients, patients with atopic dermatitis and multiple rheumatoid arthritis, patients with uveitis, and patients with more than 50% kidney function impairment. Furthermore, caution should be exercised in patients with elevated uric acid levels or hyperkalemia, rheumatic arthritis patients under 18 years of age, pregnant women or women of childbearing potential, nursing mothers, and patients taking certain medications (aliskiren, bosentan, dabigatran). Careful administration is required in elderly patients and infants under 24 months. This medication may cause various adverse reactions including impaired kidney function, hypertension, nausea, vomiting, diarrhea, loss of appetite, tremors, hirsutism, etc., which are usually dose-dependent. Due to its immunosuppressive effects, there is an increased risk of infections, and the risk of malignant tumors or lymphomas may also increase. Regular monitoring of kidney function, liver function, blood pressure, and serum drug concentrations is required during the period of taking this medication, and care should be taken regarding drug-drug interactions. Especially monitor kidney function frequently when combined with nephrotoxic drugs, and it is safe not to combine with certain medications. Pregnant or nursing mothers should avoid taking this medication, as its use during pregnancy may pose risks to the fetus. Caution is also needed when consuming potassium or using potassium-sparing medications due to the potential increase in serum potassium levels. Be careful when driving or operating machinery due to the possibility of neurological side effects from this medication. In case of overdose, symptoms may include vomiting, dizziness, headache, and impaired kidney function, and immediate consultation with medical personnel is required. This medication should be kept out of the reach of children and should not be transferred to other containers.
