Efficacy & effects

Treatment of severe liver dysfunction and hepatic encephalopathy

  • Brain function damage
  • severe liver dysfunction
  • hepatic encephalopathy
View approved original text

This medication can be used in cases of severe liver dysfunction, regardless of whether there is accompanying brain function damage due to liver failure. It is also effective in treating hepatic encephalopathy. In such cases, the administration of amino acids non-orally helps support liver function and protect brain function. Therefore, this drug is used to improve the condition in cases of liver failure or hepatic encephalopathy.

How it is given

Adult administration

Generally, intravenous drip infusion at a maximum dose of 1.5 g of total amino acids per kg of body weight daily (equivalent to 1300 mL daily for a person weighing 70 kg). The infusion rate should not exceed a maximum of 1.25 mL per kg of body weight over 60 minutes.

Precautions

This drug must be used strictly according to a doctor's instructions. It should not be administered to patients with poor kidney function or high blood nitrogen levels, patients with liver dysfunction, and those with issues in amino acid metabolism. Avoid use in patients with weak heart function, pulmonary edema, oliguria, acidosis, and those receiving excessive fluids. Do not administer to patients with a history of myocardial infarction, hyponatremia, shock, or hypoxia. Use with caution in patients with edema due to sodium retention, kidney disease, lung disease, severe heart disease history, or diabetes. Rarely, hypersensitivity reactions such as skin rash may occur when using this drug. If such symptoms appear, administration should be stopped immediately. Gastrointestinal symptoms like nausea and vomiting, as well as cardiovascular symptoms such as chest discomfort, rapid pulse, and elevated blood pressure may also occur. Administering a large volume suddenly may cause acidosis; care should be taken to avoid this. Serious adverse reactions such as chills, fever, headache, difficulty breathing, and shock have also been reported, though rarely. Various metabolic complications can occur, including metabolic acidosis, hypophosphatemia, and hyperglycemia, so they should be closely monitored. In patients with renal failure, blood BUN levels may rise when using this drug. Regular checks of blood glucose, electrolytes, fluids, acid-base balance, and liver function should be conducted during administration. This drug carries a risk of microbial contamination when mixed with other medications; therefore, it should only be mixed with products whose safety has been confirmed. Women who are pregnant or breastfeeding should only use this drug under a doctor's judgment, as its safety is not certain. In children, liver function may be immature, so concentrations should be kept low and blood ammonia levels should be checked frequently. In the elderly, body function may decline, so administration speed should be slow, and the dosage should be adjusted. Attention should be paid to electrolyte balance during administration, and it should not be infused quickly. The solution should be warmed to body temperature, and aseptic conditions for preparation and management should be maintained to reduce infection risks. Do not use if the solution is cloudy or discolored before administration; once prepared, it should be used as soon as possible and not stored in the refrigerator for more than 24 hours after preparation.
