This medication helps to reduce the side effects of antifolate drugs. It is also used in combination with 5-Fluorouracil (5-FU) to enhance treatment effects during advanced colon cancer therapy. Available in injectable and tablet forms, it can be administered according to the patient's condition. Dosage and Administration

Adult dosage

For the injectable form, when adverse reactions to methotrexate occur, administer polynate calcium 6-12 mg intramuscularly every 6 hours, four times a day. In cases of methotrexate overdose, administer an equal amount of methotrexate given. Starting 3 hours after discontinuing methotrexate, administer 15 mg intravenously every 3 hours for 9 doses, followed by 8 intravenous or intramuscular doses every 6 hours. Precautions

This medication is not suitable for treating pernicious anemia or megaloblastic anemia resulting from vitamin B12 deficiency. Caution is needed as hematological conditions may improve even if neurological symptoms continue. In the case of the injectable form, glass shards may mix in upon breaking the container, requiring extreme caution, especially when used in children or elderly patients. Pregnant or breastfeeding women should not use this medication in combination with 5-FU and should use it cautiously for toxicity reduction of antifolates. Allergic reactions such as rashes or anaphylaxis may occur, and any unusual symptoms should be reported to medical personnel immediately. There may be pain at the injection site, and rarely fever or central nervous system symptoms (paralysis, excitement, depression) may appear. When used with 5-FU, side effects such as diarrhea, dermatitis, mucositis, and leukopenia may occur, necessitating adjustment of the 5-FU dosage if symptoms are severe. This medication should be used under the guidance of a physician experienced in chemotherapy alongside methotrexate, and blood levels of methotrexate should be regularly monitored for treatment efficacy and side effect management. Patients with ascites or renal failure may have delayed drug elimination, possibly necessitating high-dose intravenous administration. As this drug shares a transporter with methotrexate, resistance to methotrexate may also imply resistance to this drug. If the patient has poor drug absorption or severe vomiting, non-oral administration is recommended, and higher doses should be administered intravenously. In particular, for patients with central nervous system tumors, repeated doses may result in drug accumulation and reduced anticancer efficacy, thus overdose should be avoided. This medication is not indicated for treatment of macrocytic anemia due to certain DNA synthesis inhibitors. It does not alleviate abnormal reactions such as nephrotoxicity caused by methotrexate and other hematologic toxicities. Due to its potential to enhance hepatic metabolism, it may lower blood concentrations of certain anticonvulsants, thus blood levels should be monitored and dosage adjustments may be necessary when given at high doses. Since it can increase the effects and side effects of 5-FU, caution is warranted when co-administering. Pregnant or breastfeeding patients should only use this drug when its benefits outweigh risks, as the impact on the fetus or newborn has not been fully elucidated. Store out of reach of children and do not transfer medications from their original containers to maintain quality and safety. When administered intravenously, care must be taken not to exceed a rate of 160 mg per minute due to calcium content, and intrathecal administration is prohibited.
