Treatment for the reduction of LDL cholesterol in hypercholesterolemia and familial hypercholesterolemia and for the reduction of cardiovascular disease risk.
This medication is primarily used as an adjunct when diet and exercise alone are insufficient for controlling primary hypercholesterolemia and mixed dyslipidemia. It also helps patients with homozygous familial hypercholesterolemia or children aged 10 to 17 years with heterozygous familial hypercholesterolemia, in combination with dietary therapy. This medication lowers total cholesterol and LDL cholesterol levels, slows the progression of atherosclerosis, and is also used for patients with primary dysbetalipoproteinemia. Additionally, it helps reduce the risk of stroke, myocardial infarction, and arterial revascularization in middle-aged patients with a high risk of cardiovascular disease. Therefore, this medication is used to improve hypercholesterolemia and associated cardiovascular disease risks.
The initial dose is 5 milligrams once daily, and if a greater reduction in LDL cholesterol is required, it can be adjusted to a maintenance dose. The maintenance dose is usually 10 milligrams once daily, and most patients are managed at this dose. The maintenance dose should be appropriately adjusted based on LDL cholesterol levels, treatment goals, and patient response every 4 weeks or longer, with a maximum daily increase of up to 20 milligrams as needed.
The usual dosage range for pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17 years) is 5 to 20 milligrams once daily. The dosage should be adjusted according to the treatment goals recommended for each individual patient.
This medication should not be used in certain patients. For example, patients with an allergy to this medication's components, with active liver disease, with muscle disorders, those with severe impairment of certain kidney functions, women who are pregnant or breastfeeding, and fertile women not using adequate contraception should not take this medication. Furthermore, this medication should be used with caution in patients at high risk of muscle injury. Elderly patients, those with hypothyroidism, patients with mild impairment of kidney function, alcohol abusers, and those with a history of certain genetic muscle disorders should consult with a physician before taking this medication. During treatment with this medication, regular liver function tests should be done, and any symptoms such as muscle pain, muscle weakness, or muscle cramps should be reported to healthcare professionals immediately. Additionally, severe muscle injury or rhabdomyolysis can occur, and if symptoms are severe, the medication should be discontinued. This medication may interact with other drugs, so it is crucial to inform healthcare professionals about any other medications being taken. In particular, the risk of muscle injury may increase when used in combination with certain antibiotics, antifungals, immunosuppressants, and fibrate medications. Pregnant or breastfeeding women should not use this medication, and fertile women should use proper contraception before taking this medication. In cases of overdose, there is no specific antidote, so one should visit a medical institution immediately for proper treatment, and liver function and muscle enzyme levels should be monitored regularly. Lastly, this medication may cause dizziness when driving or operating machinery, so caution is advised. Finally, this medication contains lactose and should not be administered to patients with lactose intolerance or related genetic disorders.
