Relief of moderate to severe acute and chronic pain
This medication is used to relieve moderate to severe acute and chronic pain.
Children aged 12 years and older and adults: The dosage should be adjusted according to the severity of the patient's pain and therapeutic response. It is recommended to start with a dose of 4 tablets, with further doses spaced at least 6 hours apart, but not exceeding 16 tablets per day. Administer the lowest effective dose for as short a duration as possible. Do not prescribe this medication for longer than necessary; if long-term use is needed due to disease characteristics and severity, regular monitoring should be conducted to confirm whether this medication should continue to be taken. Elderly: Administer the usual adult dose. However, for elderly patients aged 75 years and older, the half-life of tramadol increases by 17% when taken orally, so ensure that doses are spaced at least 6 hours apart. Renal impairment: For moderate renal impairment (creatinine clearance of 10-30 mL/min), extend the dosing interval to 12 hours. It is not recommended for severe renal impairment (creatinine clearance of less than 10 mL/min). Tramadol is removed very slowly by hemodialysis and filtration, so it does not need to be readministered after dialysis to maintain analgesic effects. Hepatic impairment: For moderate hepatic impairment, consider extending the dosing interval with caution. Do not administer this medication in cases of severe hepatic impairment.

Some patients may have a high risk of severe respiratory depression based on genetic factors affecting metabolism, necessitating genetic testing or monitoring. This medication is contraindicated in children under 12 years and adolescents under 18 who have undergone certain surgeries; avoid use in these age groups. Patients with sleep apnea or at risk for hypoxia should carefully monitor for worsening symptoms during this medication's use, and dosage may need adjustment or discontinuation.
