Treatment of hypercholesterolemia (primary, familial) and reduction of cardiovascular disease risk
This medication is primarily used as an adjunctive therapy in patients with primary hypercholesterolemia and mixed dyslipidemia when diet and exercise alone are insufficient. It can also assist in the treatment of patients with familial hypercholesterolemia and lower total cholesterol and LDL cholesterol levels, helping to slow the progression of atherosclerosis. This medication can also be used in pediatric patients aged 10 years and older who meet certain conditions. It is also utilized as an adjunct in the dietary management of patients with primary dysbetalipoproteinemia. Additionally, it aids in the reduction of the risk of stroke, myocardial infarction, and revascularization procedures in adults at high risk for cardiovascular disease. In summary, this drug is used for various conditions associated with hypercholesterolemia and the reduction of cardiovascular risk.
This medication can be taken at any time of the day, regardless of meals. The initial dose is 5 mg once daily, and if greater reduction of LDL cholesterol is required, the maintenance dosage can be adjusted accordingly. The maintenance dose for most patients is 10 mg once daily. The maintenance dose should be adjusted based on LDL cholesterol levels, treatment goals, and patient response at intervals of at least 4 weeks or more.
Pediatric patients with familial hypercholesterolemia (ages 10 to 17): Typical dosage range is 5 mg to 20 mg once daily. The dosing should be adjusted according to the individual patient's recommended treatment goals.
This medication should not be used in patients who are allergic to any of its components. It should also not be administered to patients with active liver disease or who have liver enzyme levels exceeding three times the upper limit of normal. Patients with muscle disorders or those taking cyclosporine, as well as patients with severe renal impairment, should not take this medication. Women who are pregnant or breastfeeding, and fertile women who do not use adequate contraception should not take this medication, unless they are fully aware of the risk to the fetus and the chances of pregnancy are low. Patients at high risk of muscle disorders should avoid high doses (40mg) of this medication and should exercise caution if they have factors such as renal impairment, hypothyroidism, family history of muscle disorders, alcohol abuse, or concomitant use of certain medications. This medication contains lactose, so it should not be taken by patients with lactose intolerance or related genetic disorders. Patients with a history of liver disease or those who consume excessive alcohol should undergo regular liver function tests before and during treatment. Muscle-related side effects such as myalgia, myopathy, and rhabdomyolysis may occur, so patients should inform their doctor immediately if they experience muscle pain, weakness, or cramps. Special attention should be given to elderly patients, those with renal impairment, hypothyroidism, or those with alcohol abuse due to a higher risk of muscular side effects. In cases of severe acute illness, this medication should be discontinued, and if severe muscle weakness or ophthalmoplegia occurs, it should be stopped immediately. Concomitant use of this medication with certain drugs (e.g., fibrates, cyclosporine, some antibiotics) may increase the risk of muscle toxicity, so caution is advised when using these combinations. High-dose administration may cause temporary proteinuria, so renal function tests should be considered. Asian patients may have higher serum concentrations of this medication, so dosage adjustments may be necessary. Monitoring of growth and secondary sexual characteristics is recommended in pediatric and adolescent patients. Rare side effects such as interstitial lung disease, severe skin reactions, liver function abnormalities, and the possibility of diabetes should be discussed with a healthcare professional if symptoms occur. Additionally, this medication can interact with various other drugs, so patients must inform their physician or pharmacist if they are taking other medications. Pregnant or breastfeeding women should not take this medication, and fertile women should use appropriate contraception. In the case of overdose, while there is no specific antidote, treatment will depend on the symptoms and liver and muscle function tests will be necessary. Caution is advised when driving or operating machinery as dizziness may occur.
Allergic reactions including urticaria, angioedema, pancreatitis, myopathy, rhabdomyolysis, severe muscle weakness, ophthalmoplegia, Stevens-Johnson syndrome, drug reaction with eosinophilia and systemic symptoms (DRESS), immune-mediated necrotizing myopathy, jaundice, hepatitis, liver dysfunction, acute renal failure, oliguria, severe edema accompanied by elevated blood pressure, severe rash with high fever, blisters, skin peeling, oral/ocular lesions
