This medication is used in patients with primary hypercholesterolemia and mixed dyslipidemia when diet and exercise alone do not adequately control lipid levels. It may also assist patients with homozygous familial hypercholesterolemia when used in conjunction with dietary therapy or other lipid-lowering therapies. It is effective in lowering total cholesterol and LDL cholesterol levels in patients with hypercholesterolemia, thereby helping to slow the progression of atherosclerosis. It is also used as an adjunct to dietary therapy for pediatric patients who meet specific criteria for heterozygous familial hypercholesterolemia. Additionally, it aids in dietary management for patients with primary dysbetalipoproteinemia. Furthermore, it is effective in reducing the risk of stroke, myocardial infarction, and arterial revascularization in adults at high risk for cardiovascular diseases. This medication improves dyslipidemia and the associated risk of cardiovascular diseases.

How to take

Adult dose

For patients with primary hypercholesterolemia and other dyslipidemias, it is usually taken once daily at any time regardless of meals. The initial dose is 5 milligrams and may be increased up to 20 milligrams at intervals of at least 4 weeks as needed. The typical maintenance dose is 10 milligrams once daily.

Child dose

For pediatric patients (ages 10 to 17) with heterozygous familial hypercholesterolemia, the dosage is 5 milligrams to 20 milligrams once daily. Dosage should be adjusted individually according to treatment goals at intervals of at least 4 weeks.

Precautions

This medication should not be used in certain patients. First, individuals who have had an allergic reaction to this medication must not take it. Patients with active liver disease or significantly elevated liver enzyme levels should also avoid this product. Patients with muscle disorders, those taking cyclosporine, or those with severely impaired renal function should refrain from using this medication. Special caution is required for pregnant or breastfeeding women and for women of childbearing age not using adequate contraception; it should only be used if the potential risk to the fetus is thoroughly explained. High-dose administration is particularly prohibited in patients at risk for myopathy or rhabdomyolysis. Additionally, patients with lactose intolerance or related genetic disorders should not take this medication. This medication should be used cautiously in patients with a history of alcohol overconsumption or liver disease, with frequent liver function tests conducted. If there are underlying conditions such as hypothyroidism or nephrotic syndrome, these conditions should be addressed prior to treatment. Muscle-related side effects may rarely occur; therefore, if muscle pain, weakness, or cramps occur, the patient should notify the physician immediately, and blood tests may be necessary to check for muscle damage. Particularly elderly patients, those with renal impairment, or those with a history of alcohol abuse, who are at higher risk for muscle side effects, should be closely monitored. This medication can also affect liver function, potentially increasing liver enzyme levels, so regular liver function testing is recommended. Rarely, severe liver damage or skin reactions may occur; if symptoms such as skin rash or jaundice appear, the patient should stop taking the medication and consult their doctor immediately. This medication may interact with other drugs; therefore, any co-administered medications should be reported to the healthcare provider. Specifically, concomitant use with some antiviral agents, immunosuppressants, antifungals, antibiotics, and fibrate-class drugs may increase the risk of muscle side effects. Moreover, when taken with antacids, this medication's absorption may decrease, so spacing the dosing is advisable. Pregnant or breastfeeding women should not take this medication, and women of childbearing age should employ appropriate contraceptive methods. If taken during pregnancy, it may be harmful to the fetus, and usage should cease immediately upon confirmation of pregnancy. In the case of overdose, there is no specific antidote, thus treatment should focus on symptomatic relief and monitoring of liver function and muscle enzyme levels. This medication is not known to significantly affect driving or operating machinery, but caution should be exercised as dizziness may occur. Finally, this medication may show higher concentrations in Asian individuals, possibly necessitating dose adjustments. Although its efficacy and safety in pediatric patients is similar to that in adults, monitoring for growth and secondary sexual characteristics is required.
