Efficacy & effects
This medication helps prevent seizures due to hypomagnesemia and alleviate seizures caused by eclampsia. It also prevents uterine contractions and promotes electrolyte replenishment in hypomagnesemic states. It is effectively used to improve patients' symptoms through these effects.
Dosage
Seizures due to hypomagnesemia: administer 1 to 2 g of magnesium sulfate intravenously (4.05 to 8.1 mmol). Seizures of eclampsia: administer 4 to 5 g intravenously over 10 minutes (16.2 to 20.25 mmol). Prevention of uterine contractions: administer an initial 4 g intravenously (16.2 mmol). Subsequent doses of 1 g can be given. Electrolyte replenishment (hypomagnesemia): administer 1 g via intramuscular injection 4 times every 6 hours.
Precautions
This medication is in a glass ampule formulation, so glass shards may mix when cutting the container, and utmost caution should be exercised during use. Care should be taken especially when using it for children and the elderly. This medication should not be administered to patients with poor kidney function, comatose patients, patients with high magnesium levels, patients with cardiac conduction disorders, and patients with myasthenia gravis. Caution is required when administering this medication to children with intestinal parasitic diseases or patients with heart diseases. While using this medication, gastrointestinal issues such as vomiting or dry mouth may occur, and rarely, symptoms like arrhythmias, vasodilation, headache, tremors, and tingling may also occur. Additionally, in case of overdose, magnesium toxicity may occur, which can present as heat sensation, thirst, hypotension, central nervous system depression, bradycardia, and respiratory paralysis, thus caution is advised. In such cases, calcium salts should be administered intravenously for detoxification. This medication may interact with other drugs, so caution is necessary when used with barbiturates, narcotics, sedatives, iron supplements, certain antibiotics, and muscle relaxants. During pregnancy, especially for pregnant women with eclampsia, it may be administered to protect the life of both the fetus and the mother; however, side effects such as decreased fetal muscle tone, diminished reflexes, hypotension, and respiratory depression in newborns may occur, so fetal status should be monitored. If breastfeeding, the safety of this medication has not been established, and it is advisable not to use it unless absolutely necessary. Elderly patients may have decreased physiological function, so the administration speed should be slow and the dose should be reduced for careful use. In case of overdose, high magnesium levels in the blood can lead to dangerous symptoms such as arrhythmias, respiratory depression, and cardiac arrest, so administration should be stopped immediately, and if necessary, detoxification should be performed with calcium gluconate injections. This medication should be slowly infused at a concentration of less than 10% when administered intravenously, and care should be taken as precipitation may occur when mixed with other drugs. Additionally, to prevent infections, the injection site and equipment should be thoroughly sterilized, used immediately after opening, and leftover medication should not be reused.
