Treatment of mood disorders (depression and premenstrual dysphoric disorder), obsessive-compulsive disorder, and bulimia nervosa
This medication is used to treat patients with depression, and symptom improvement effects generally appear within 5 to 6 weeks. Depression includes symptoms such as persistent sadness, changes in appetite or sleep, psychomotor agitation or retardation, loss of interest, fatigue, feelings of guilt, impaired concentration, and suicidal thoughts. Additionally, it effectively reduces binge eating and purging behavior in patients with neurogenic appetite increase. For obsessive-compulsive disorder, it reduces obsessive thoughts and behaviors, alleviating the distress related to social and work life. It is also effective for premenstrual dysphoric disorder, improving cyclic symptoms such as depression, anxiety, and physical pain. This medication is used to treat these various mental health issues.
Adults 1. Depression 1) Initial treatment: The initial dose is 20mg of fluoxetine hydrochloride orally once daily in the morning. In controlled clinical trials, doses of 20~80mg daily were administered. If no improvement is observed, an increase in the dose may be considered after several weeks. Doses exceeding 20mg daily should be administered in 2 divided doses (morning and noon), and the maximum dose of 80mg daily should not be exceeded. The onset of an adequate antidepressant effect may be delayed until 4 weeks or longer after starting treatment. 2) Maintenance/continuation/extended treatment: There are no definitive data on how long patients should be treated with this drug, but psychopharmacologists recognize that acute episodes of depression require drug treatment for several months or longer. It is not known whether the amount of antidepressant needed to relieve symptoms is the same as that needed to maintain or sustain remission. 2. Bulimia nervosa Administer 60mg of this drug daily. The daily dose should not exceed 80mg. 3. Obsessive-compulsive disorder Administer 20~60mg of this drug daily. The daily dose should not exceed 80mg. 4. Premenstrual dysphoric disorder Administer 20mg of this drug daily. Reassess the need for continued treatment after 6 months of treatment. Patients with renal and/or hepatic impairment, patients receiving multiple medications, elderly patients, and patients with comorbid diseases: Administer a lower dose or reduce the frequency of administration. Adjust the dose as appropriate according to age and symptoms. Switching from MAO inhibitors used to treat psychiatric disorders When discontinuing an MAO inhibitor used to treat a psychiatric disorder and starting treatment with this drug, allow an interval of at least 14 days. Conversely, when starting an MAO inhibitor to treat a psychiatric disorder, at least 5 weeks must have elapsed after discontinuing this drug. Other MAO inhibitors, such as linezolid or methylene blue Patients receiving linezolid or intravenous methylene blue should not start treatment with this drug because of the increased risk of serotonin syndrome. For patients requiring treatment of a more urgent psychiatric condition, including hospitalization, other interventions should be considered. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this drug. If no alternative to linezolid or intravenous methylene blue is available and the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome in a particular patient, this drug may be discontinued immediately and linezolid or intravenous methylene blue administered. The patient should be monitored for symptoms of serotonin syndrome for 5 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever comes first. Treatment with this drug may be resumed 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by non-intravenous routes (oral tablets or local injection) or with intravenous administration of this drug at a dose of 1mg/kg or less is unclear.
This medication is primarily used to treat symptoms associated with depression, and special caution is required in children or adolescents and young adults, as the risk of suicidal thoughts or behaviors may increase. Before taking this medication, it should not be used if there is a history of hypersensitivity, severe renal impairment, or if the patient is taking specific types of medications, such as MAO inhibitors. Patients with a history of seizures may have an increased risk of seizures while taking this medication, so caution is needed, and patients at risk for suicide should be closely monitored during the initial treatment and when changing dosages. Patients with heart diseases or those at risk of cardiac rhythm disorders should also undergo ECG monitoring and careful observation while using this medication. Patients with impaired liver or kidney function might need dose adjustments due to the potential effects on drug metabolism and excretion. Diabetes patients should regularly monitor their blood glucose levels as there may be changes in blood glucose control. This medication may cause pupil dilation, and caution is advised for patients with high intraocular pressure or at risk for glaucoma. Common side effects include headaches, nausea, insomnia, fatigue, and diarrhea, while rare but severe allergic reactions or serious skin reactions may occur. Discontinuing this medication abruptly may result in withdrawal symptoms such as dizziness, sensory disturbances, sleep disturbances, anxiety, nausea, tremors, etc., thus dosage should be gradually reduced under medical supervision. This medication may interact with other drugs, so patients taking other medications must inform their doctor or pharmacist, especially since the combined use with anticoagulants may increase bleeding risk. During pregnancy, the risks to the fetus should be carefully weighed against the benefits of the medication, and the same caution applies during breastfeeding since it may be excreted in breast milk and affect the baby. Safety and efficacy in children and adolescents have not been sufficiently established, so it's preferable to avoid prescribing it to them. Elderly patients may have an increased risk of side effects due to comorbidity or polypharmacy, thus caution is warranted. In cases of overdose, serious cardiovascular and central nervous system issues may occur, requiring immediate medical attention, as there is no specific antidote, so symptomatic treatment is necessary. Lastly, this medication should be kept out of reach of children, and transferring it to another container should be avoided.
