Efficacy & effects

Improvement of hypoalbuminemia and malnutrition

  • Hypoalbuminemia
  • malnutrition
View approved original text

Hypoalbuminemia, malnutrition, and situations such as pre- and post-surgery where amino acid supplementation is necessary. In such cases, appropriate supplies of amino acids play a crucial role in protein synthesis and tissue recovery, helping to maintain health and facilitate recovery. Hence, amino acid administration is useful in such situations and is used to improve nutritional status and promote recovery.

Dosage and Administration

Administer by dripping a maximum dose of 1 to 1.5 g per day per kg of body weight as total amino acids. The infusion rate should ideally provide a total concentration of about 10 g of total amino acids over 60 minutes, with the principle of administering 30 to 40 drops per minute.

Precautions

This medication must be used strictly according to a physician's instructions. It should not be used in patients with hepatic coma symptoms or those at risk of such. Patients with severe renal impairment or high blood urea nitrogen levels should also avoid administration. This drug is not suitable for patients with nitrogen metabolism issues. It should not be administered to patients with cardiac and pulmonary diseases such as heart failure or pulmonary edema. This medication should not be used in patients with extremely low or absent urine output. If blood sodium and chloride levels are high, especially in products containing sodium and chloride ions, use should be avoided. Cautious administration is required in patients with severe acidosis, congestive heart failure, edema due to sodium retention, elevated potassium levels or potassium retention in the body, or liver or kidney dysfunction. Allergic reactions such as rash may occur when using this drug, and immediate discontinuation is necessary if such symptoms arise. Occasionally, gastrointestinal symptoms like nausea or vomiting may appear. Palpitations or chest discomfort may occur, and blood pressure could rise. Rapid infusion of large amounts may pose a risk of acidosis. Serious side effects such as chills, fever, headache, respiratory distress, and shock may also occur, necessitating careful monitoring. Metabolic acidosis, hypophosphatemia, alkalosis, hyperglycemia, diabetes, dehydration, hypoglycemia, elevated liver enzymes, vitamin deficiencies or excesses, and electrolyte imbalances could happen. Patients with poor liver or kidney function may see increased blood urea nitrogen (BUN) levels. Patients with very poor liver function may experience amino acid imbalance in the bloodstream or metabolic alkalosis, as well as coma states. This drug can influence the immune system, posing a risk of serious allergic reactions such as anaphylactic shock. When administered in high doses, electrolyte balance should be monitored, and when used with electrolyte-containing solutions, particular caution is required. Overdosage may lead to metabolic alkalosis, thus formulations containing acetate ions should be used carefully. Patients with a history of kidney disease, pulmonary disease, or heart disease must adjust dosage and infusion rates to prevent side effects due to fluid overload. Diabetic patients should frequently monitor blood sugar levels. Caution is needed for interactions when used with other medications, particularly with tetracycline antibiotics, as they may reduce protein-sparing effects. During pregnancy, the safety of this medication has not been adequately confirmed, and it should only be used if the benefits outweigh the risks. Children and neonates may have immature liver function, requiring lower infusion concentrations and frequent monitoring of blood ammonia levels. Elderly patients may experience decreased bodily function, so it is advisable to slow the infusion rate and adjust dosages accordingly. This medication contains electrolytes, thus electrolyte balance must be meticulously managed during administration. Infusion should be performed slowly intravenously, as rapid injection may cause chills or vomiting and lead to amino acid loss. Before use, the solution should be warmed to body temperature, and preparation and management should be done under sterile conditions to reduce infection risk. If the solution is discolored or contains leakage or precipitate, it should not be used. During storage, the packaging should not be opened, and once opened, it should be used promptly.
