Treatment of hypertension (symptomatic, resistant)
This medication is used for hypertension caused by symptomatic or specific organ damage. It is also used for the treatment of resistant hypertension that is ineffective when combined with diuretics and two antihypertensive medications. Therefore, it helps manage hypertension and improves blood pressure control.
For adults (12 years and older), the initial dose of minoxidil is taken orally as 5 mg once daily. The maintenance dose is 10-40 mg divided into 1-4 times daily, with a maximum of 100 mg per day. The frequency of administration is adjusted according to fluctuations in arterial blood pressure; if the reduction in preload is less than 30 mmHg, it can be administered once daily; if it is 30 mmHg or more, it is divided into twice daily.
For children (under 12 years old), an initial dose of 0.2 mg per kg of body weight is administered once daily, and the dose can be increased by 50-100% until blood pressure is optimally adjusted. As a maintenance dose, 0.25-
This medication should not be used in patients with certain conditions. For example, patients with pheochromocytoma, pulmonary hypertension due to mitral stenosis, acute myocardial infarction, or those who have had allergic reactions to this medication should not be administered. Additionally, patients with chronic congestive heart failure, severe renal impairment, or angina should use this medication with special caution. Using this medication may commonly lead to abnormal hair growth, or hypertrichosis. This phenomenon typically occurs within 3-6 weeks after starting treatment, primarily beginning on the face and head and can spread throughout the body. Discontinuation of administration halts new hair growth, but it may take several months to return to the original state. Skin rashes, vesicles, or severe allergic reactions may also occur; therefore, if abnormal symptoms appear, medical personnel should be informed immediately. Blood tests may show a decrease in platelet or white blood cell counts, and liver function-related levels may also change. Electrocardiograms may show changes in cardiac electrical signals in some patients, but most recover with continued treatment. Pericardial effusion, where fluid accumulates around the heart, can rarely occur, and additional tests and treatments may be necessary in such cases. Other side effects, such as weight gain, peripheral edema, increased heart rate, gastrointestinal disturbances, and headaches, may rarely occur. When using this medication, it is essential to manage the body's sodium and fluid balance carefully, and diuretics are often used together to prevent fluid retention. Vascular dilation may increase heart rate; therefore, other medications may be used concurrently for control. Patients with poor renal function may need to adjust the dosage, and this medication should only be used when the patient is stabilized after a myocardial infarction. Initial administration requires regular monitoring through urinalysis, kidney function tests, electrocardiograms, and chest X-rays to closely observe the condition. This medication can interact with other drugs, so caution is needed, especially when taking antihypertensives or psychiatric medications together. It is advisable not to use this medication during pregnancy or breastfeeding, and if it must be used, breastfeeding should be discontinued. In children, especially newborns, there is limited experience in use, so administration should be done with great caution. In the case of overdose, immediate contact with a medical institution for appropriate measures is required, and treatment to maintain blood pressure and protect kidney function may be necessary.
