Blood sugar control in type 2 diabetes and prevention of cardiovascular events
This drug is used as a treatment to help adults with type 2 diabetes control their blood sugar levels by combining empagliflozin and metformin. It is especially effective when administered as an adjunct to dietary control and exercise therapy, effectively improving blood sugar levels. It may also be used in patients with type 2 diabetes who have cardiovascular disease, but it is important to consider the risks of cardiovascular events as per expert recommendations. Therefore, this drug helps control blood sugar and shows positive effects in diabetes treatment.
The recommended dosage of this medication is 1 tablet, twice a day. The dosage is determined considering the treatment regimen, efficacy, and tolerability of each patient, without exceeding the maximum daily dose of 25 mg empagliflozin and 2000 mg metformin. In cases where blood sugar control is insufficient using metformin alone or in combination with other medications such as insulin, the recommended dose is 5 mg empagliflozin twice daily along with the existing dose of metformin. This medication may cause a dangerous condition known as severe lactic acidosis, so precautions are necessary particularly in patients with impaired kidney function or weak heart or liver function. When used with insulin or sulfonylureas, hypoglycemia may occur, so blood sugar changes should be closely monitored while using together. This drug should not be used in patients with moderate to severe renal impairment or those in situations that may lead to acute kidney injury, and should also be avoided during acute illnesses such as dehydration, severe infections, or acute myocardial infarction. Additionally, it should not be used in patients with type 1 diabetes, a history of diabetic ketoacidosis, unstable or acute heart failure, or women who are pregnant or breastfeeding. Before and after procedures using iodinated contrast agents, this drug should be discontinued due to the risk of worsening kidney function and lactic acidosis, and it should only be resumed after ensuring normal kidney function. Regular kidney function tests should be conducted during treatment, especially in the elderly or those with slightly decreased kidney function who may require more frequent assessments. Caution should be exercised in patients at risk of fluid loss or those prone to dehydration due to gastrointestinal disturbances; in such cases, temporary discontinuation of medication may be necessary. This drug may cause side effects such as hypoglycemia, urinary infections, genital infections, increased urination, gastrointestinal disturbances (nausea, vomiting, diarrhea, etc.), so consultation with healthcare professionals should occur upon symptom onset. It is safest for women who are pregnant or may become pregnant and nursing mothers not to use this medication. If breastfeeding, it should be discontinued. Safety and efficacy in pediatric patients have not been established; thus, children under 18 years of age should not use this medication. Elderly patients are at higher risk for decreased renal function and should have their dosage adjusted and renal function regularly monitored. In cases of overdose, appropriate treatments such as hemodialysis may be necessary, and immediate medical attention should be sought if symptoms appear.
