In summary, this medication is effective for essential hypertension, heart failure (especially in patients intolerant to ACE inhibitors), and reducing the risk of death after myocardial infarction. Therefore, it is used to improve cardiovascular health.
Essential Hypertension: Take once daily with water either during meals or in between meals. It is recommended to take 80 mg orally at the same time each day if possible. If blood pressure control is inadequate, the dose may be increased to 160 mg per day or combined with a diuretic, with a maximum daily dose of 320 mg. Elderly patients (aged 75 years and older) should start at 40 mg once daily. Heart Failure: Take 4 mg twice daily.
Safety and efficacy have not been established in patients under the age of 18.

Patients with heart valve disease or hypertrophic cardiomyopathy, and those with ischemic heart disease or cerebral vascular disease should be careful to avoid excessive drops in blood pressure. When first taking this medication, blood pressure may drop rapidly, causing dizziness or fainting, so particularly those with low fluid or sodium levels should start at a low dose and gradually increase. Patients with renal artery stenosis may experience renal function deterioration, so renal function should be monitored closely while taking this medication. Patients with heart failure or after myocardial infarction may experience changes in kidney function, so regular kidney function tests are important. This medication lowers blood pressure, so caution is advised while driving or operating machinery as dizziness or drowsiness may occur. Taking this medication with potassium-sparing diuretics or potassium supplements may lead to dangerously high potassium levels, so consultation with a physician is necessary when taking together. When taken with non-steroidal anti-inflammatory drugs (NSAIDs), the blood pressure-lowering effect may be reduced and renal function may deteriorate, so caution is advised. Taking it with lithium preparations may increase lithium levels and result in toxicity, so monitoring of blood lithium levels is needed. In case of overdose, blood pressure may drop very low, requiring immediate medical attention for appropriate treatment.
