This medication is used during bicarbonate dialysis for patients with acute and chronic renal failure, and also for preparing dialysis solutions necessary for detoxification of toxic substances.

Dosage and Administration

Adult dosage

1. When using the specified acetic acid dialysis solution (A solution), this preparation can create about 180L of bicarbonate dialysis solution containing 38 mEq/L of HCO3-. 2. When using the dialysis solution for bicarbonate dialysis with a dilution ratio of A solution 1: (this drug + purified water) 34, it is used for blood dialysis. 3. For each patient per session (based on 6 hours) of dialysis solution

Precautions

This drug should not be used in patients with very low blood pressure or rapid pulse. Patients with pulmonary edema or significant ECG changes should also not receive this drug. Use in patients with elevated serum potassium levels or coagulation issues may also be dangerous. Caution is advised in patients with severely impaired liver function or metabolic disturbances due to diabetes. Particularly, patients on digitalis preparations should be careful of intoxication risks due to changes in potassium levels. In patients with high serum calcium and phosphorus levels before dialysis, this drug may cause ectopic calcification, so caution should be exercised. Patients with insufficient calorie intake or dietary restrictions due to diabetes should be monitored for hypoglycemia risk. Hypoglycemia may occur with this drug, so appropriate measures such as glucose injection or sugar supplementation should be taken if symptoms of low blood sugar arise. Serum calcium levels may increase, requiring management such as adjusting the dialysis solution if hypercalcemia is suspected. Bone-related complications like osteoporosis, osteomalacia, and fibrous osteitis may occur, necessitating treatment with vitamin D3 preparations. It is crucial to maintain serum phosphorus levels within the normal range to prevent ectopic calcification. If headaches, nausea, vomiting, convulsions, or altered consciousness occur during dialysis, measures such as reducing the dialysis speed may be necessary. This drug is used for dialysis in patients with chronic renal failure and may be selectively used when acetic acid dialysis is challenging to manage. As it is a dialysis solution that does not contain glucose, it may be beneficial when blood sugar control is difficult in patients with glucose metabolism disorders. Regular blood tests to monitor electrolytes, acid-base balance, kidney function, and blood sugar are important. Safety during pregnancy has not been established, so women of childbearing potential should use this drug cautiously under medical supervision. The safety in pediatric patients has also not been sufficiently confirmed, requiring caution. Elderly patients should understand the characteristics of this drug well, and regular examination and treatment may be required to prevent bone complications with prolonged use. The water used for dialysis should be safe water subjected to strict purification processes, and precautions regarding potential membrane damage and bleeding risk must be observed. When preparing dialysis solutions, sodium bicarbonate solutions and other electrolyte solutions should not be mixed directly, but rather diluted accurately at defined concentrations. If the concentration is too high or low, side effects such as headaches, palpitations, blood pressure abnormalities, or altered consciousness may occur, thus caution is necessary. The electrolyte concentration and pH of the dialysis solution must be checked before use, and it should be applied immediately after preparation. The temperature should be adjusted to body temperature during use, and it should not be used for injection or peritoneal dialysis. Only cartridges compatible with specific dialysis systems should be used, and caution should be exercised to avoid overpressure or negative pressure exposure.
