Treatment of major depressive disorder, anxiety disorders (including panic disorder and social anxiety disorder), and obsessive-compulsive disorder
This medication is used for the treatment of major depressive disorder and panic disorder (including those with or without agoraphobia), social anxiety disorder (social phobia), generalized anxiety disorder, and obsessive-compulsive disorder.
The safety of doses exceeding 20mg per day has not been established. This medicine is administered once daily, in the morning or evening, regardless of meals. This medicine is an orally disintegrating tablet. Place it on the tongue and allow it to dissolve, then take it with or without water. Major depressive disorder: The usual dose is 10mg once daily. The dose may be increased to a maximum of 20mg per day according to the patient's response. If increasing the dose of this medicine to 20mg, allow an interval of at least 1 week before increasing the dose. Antidepressants generally take 2-4 weeks to produce an effect. Even after symptoms have resolved, at least 6 months of drug treatment is required to ensure a therapeutic effect. Panic disorder with or without agoraphobia: It is recommended to administer an initial dose of 5mg per day for 1 week, then increase to 10mg per day. The dose may be increased to a maximum of 20mg per day according to the patient's response. The maximum effect occurs after approximately 3 months. The safety and efficacy of long-term administration of this medicine for more than 10 weeks in the treatment of panic disorder have not been systematically studied. The physician should periodically reassess each patient to determine the need for continued treatment. Social anxiety disorder: The usual dose is 10mg once daily. Symptom improvement generally takes 2-4 weeks. Thereafter, the dose may be reduced to 5mg per day or increased to a maximum of 20mg per day according to the patient's response. Social anxiety disorder is a chronic condition, and continued treatment for 12 weeks is recommended to ensure a therapeutic effect. Long-term treatment of responding patients has been studied for 6 months and should be considered on an individual patient basis to prevent relapse. The usefulness of long-term treatment should be reassessed regularly. Generalized anxiety disorder: The initial dose is 10mg once daily. The dose may be increased to a maximum of 20mg per day according to the patient's response, after at least 1 week of treatment. A long-term study was conducted in patients who responded to 12 weeks of open-label treatment, administering 20mg per day for at least 6 months. The physician should regularly reassess the usefulness of long-term treatment with this medicine for each patient. Obsessive-compulsive disorder: The usual dose is 10mg once daily. The dose may be increased to a maximum of 20mg per day according to the patient's response. A long-term study was conducted in patients who responded to 16 weeks of open-label treatment, administering 10mg or 20mg per day for at least 24 weeks. Obsessive-compulsive disorder is a chronic condition; treatment should be continued for a sufficient period to ensure symptom remission. When this medicine is used long term, the usefulness and dose should be regularly reassessed for each patient. Patients with renal impairment: No dose adjustment is required in patients with mild or moderate renal impairment. Caution should be exercised in patients with severe renal impairment (CLCR < 30㎖/min). Patients with hepatic impairment: An initial dose of 5mg per day for the first 2 weeks is recommended. The dose may be increased to 10mg per day according to the patient's response. Discontinuation: When discontinuing this medicine, the dose should be gradually reduced over at least 1-2 weeks to avoid the possibility of withdrawal symptoms. Switching from MAO inhibitors for the treatment of psychiatric disorders: At least 14 days should elapse after discontinuing an MAO inhibitor used to treat psychiatric disorders before starting treatment with this medicine. Conversely, at least 14 days should elapse after discontinuing this medicine before starting an MAO inhibitor for the treatment of psychiatric disorders. Other MAO inhibitors such as linezolid or methylene blue: Patients receiving linezolid or intravenous methylene blue should not start treatment with this medicine because of the increased risk of serotonin syndrome. For patients requiring more urgent treatment of a psychiatric condition, including hospitalization, other interventions should be considered. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this medicine. If no alternative to linezolid or intravenous methylene blue is available and the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome in a particular patient, this medicine may be discontinued immediately and linezolid or intravenous methylene blue administered. The patient should be monitored for symptoms of serotonin syndrome for 2 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever comes first. Treatment with this medicine may be restarted 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by non-intravenous routes (oral tablets or local injection) or intravenous administration of this medicine at doses of 1mg/kg or less is unclear. Nevertheless, clinicians should be aware of the possibility of emergency symptoms of serotonin syndrome with these uses.
This medication is an antidepressant used to treat psychiatric disorders such as major depression. Caution is required as there may be an increased risk of suicidal thoughts and behaviors in children, adolescents, and young adults. This medication contains the artificial sweetener aspartame, so it should not be taken by patients with phenylketonuria. Patients with hypersensitivity to any of the ingredients of this drug or currently taking certain medications (MAO inhibitors, Pimozide, etc.) should not use it. Patients with closed-angle glaucoma or a history of glaucoma should be cautious as this medication may increase eye pressure. During the initial 1-2 weeks of treatment, mental symptoms such as anxiety, insomnia, and suicidal thoughts may occur, so monitoring in consultation with a physician is required. Sudden discontinuation may cause withdrawal symptoms like dizziness, anxiety, and sleep disturbances, so it should be tapered off gradually according to a physician's instructions. This medication may cause arrhythmias or prolong the QT interval, so patients with heart diseases should use it with caution. In combination with other serotonergic medications, a severe side effect known as serotonin syndrome may occur, so concurrent use should be avoided. Pregnant women should use it with caution as it may affect the fetus, and nursing mothers should consider discontinuing breastfeeding. Older adults may need to take a reduced dose for safety. Caution should be exercised when driving or operating machinery according to symptoms. In case of overdose, symptoms such as dizziness, tremors, vomiting, and arrhythmias may occur, and immediate medical attention is required. Keep out of reach of children and do not transfer to other containers.
