This medication is used to help control blood sugar levels in patients with type 2 diabetes. In particular, the combination of sitagliptin and metformin is effective in improving blood sugar levels, especially when used in conjunction with dietary and exercise regimens.

How to take

Adult dose

The dosage of this medication should be determined based on the patient’s current therapy, efficacy, and tolerability, ensuring that it does not exceed the maximum daily recommended doses of sitagliptin 100 mg and metformin 2000 mg. This medication should be taken once daily, at dinner, to reduce gastrointestinal side effects caused by metformin, and the dose may be gradually increased. Do not split, crush, chew, or dissolve the extended-release tablet.

Precautions

This medication is composed of sitagliptin and metformin and is used for the treatment of diabetes. First, sitagliptin may rarely cause serious allergic reactions; if symptoms such as rash, hives, or difficulty in breathing occur, discontinue use immediately and consult a physician. Additionally, sitagliptin can potentially trigger acute pancreatitis, and symptoms such as abdominal pain, vomiting, or nausea should be reported to medical personnel immediately. Metformin can cause a rare but life-threatening adverse effect known as lactic acidosis; thus, if sudden shortness of breath, muscle pain, fatigue, or abdominal pain occur, discontinue the medication and visit a hospital immediately. This medication should not be administered to patients with moderate to severe renal impairment, acute heart failure, acute myocardial infarction, severe infections, or dehydration. If tests using contrast agents for imaging studies are planned, this medication should be discontinued during that period, and it should be resumed only after confirming normal kidney function. This medication is not recommended for use in pregnant women or women who may become pregnant, nor in nursing mothers. There is a risk of hypoglycemia while taking this medication; therefore, regular meals should be observed, and sudden exercise or skipping meals should be avoided. Extra caution is necessary when used with other glucose-lowering agents. Also, alcohol consumption can increase the risk of lactic acidosis, so it should be avoided while taking this medication. For those undergoing surgery, this medication should be discontinued 48 hours prior to the operation and should only be resumed after confirming that oral intake has normalized and kidney function is stable. Gastrointestinal disturbances such as diarrhea, nausea, vomiting, and abdominal pain may occur while using this medication, and patients should consult a physician if these symptoms are severe. Regular monitoring of kidney function and blood tests are necessary to check for vitamin B12 deficiency due to metformin. Elderly patients or those with decreased kidney function may require dosage adjustments and more frequent monitoring of kidney function. If any unusual symptoms or suspected adverse effects occur while taking this medication, medical personnel should be notified immediately, and the medication should not be discontinued or the dose altered arbitrarily.

Mild side effects

Diarrhea, upper respiratory infection, headache, nausea, vomiting, abdominal pain, nasopharyngitis, muscle pain, limb pain, back pain, pruritus, rhabdomyolysis, thrombocytopenia, peripheral edema, hypoglycemia, severe and disabling joint pain, bullous pemphigoid, elevated liver enzymes, slight increase in white blood cell count, increase in serum creatinine, decrease in vitamin B12
