Summary

Main use
Treatment of obesity (weight loss and maintenance); prevention of weight regain
Who takes it
Adults · Children
Adult dose
○ Adults The recommended dose of this medicine is 120 mg of orlistat orally 3 times daily, taken with a fat-containing meal or within 1 hour after a meal. Patients should continue a nutritionally balanced, mildly hypocaloric diet in which approximately 30 % of calories are derived from fat. Daily intake of fat, carbohydrates, and protein should be distributed evenly among three meals. This medicine may be omitted if a meal is missed or contains no fat. Because this medicine may reduce the absorption of some fat-soluble vitamins and beta-carotene, patients are advised to take a multivitamin containing fat-soluble vitamins to ensure adequate nutrition. The vitamin supplement should be taken at least 2 hours before this medicine or at least 2 hours after it, such as at bedtime. Taking more than 120 mg of this medicine 3 times daily does not increase its effectiveness. Based on measurements of fecal fat, the effect of this medicine appears 24 ∼ 48 hours after administration. After treatment is discontinued, fecal fat content generally returns to pretreatment levels within 48 ∼ 72 hours. The efficacy and safety of this medicine for periods exceeding 4 years have not been evaluated. ○ Studies have not been conducted in patients with hepatic and/or renal impairment or in older adults.
Child dose
Studies have not been conducted in children under 12 years of age.

Precautions

This medication should not be used in patients with chronic malabsorption syndrome or biliary obstruction. Patients who are allergic to this medication or any of its components should also avoid taking it. Women who are pregnant, potentially pregnant, or breastfeeding should not take this medication. Since it contains lactose, it should not be administered to patients with lactose intolerance or certain genetic disorders. Patients with liver impairment should use this medication cautiously and immediately inform their physician if symptoms of liver dysfunction appear. Patients with a history of hyperoxaluria or calcium oxalate kidney stones should also use this medication carefully. Adverse reactions mainly occur in the gastrointestinal tract, and symptoms such as abdominal distension, fatty stools, and increased bowel movements are commonly reported. Adverse reactions are generally mild and temporary, typically starting within 3 months and decreasing over time. Discontinuation of treatment may occur due to gastrointestinal adverse reactions; thus, consultation with a physician is necessary if symptoms are severe. Taking this medication with a high-fat meal may increase the frequency of gastrointestinal adverse reactions, so it's advisable to adjust fat intake while using it. This medication may interfere with the absorption of fat-soluble vitamins and beta-carotene, thus taking a multivitamin supplement is recommended. If symptoms of liver dysfunction (loss of appetite, jaundice, itching, etc.) occur, treatment should be stopped immediately, and a hospital visit is necessary. Diabetic patients may require adjustments to their diabetes medication dosage due to weight loss from this medication, so they should consult their physician. Excessive weight loss may increase the risk of gallstones, so caution is advised. This medication may interact with other drugs, so patients, especially those taking anticoagulants, cyclosporine, or antiepileptic medications, should inform their physician. Women who are pregnant or breastfeeding should not use this medication, and it is not recommended for children under 12 years of age. Due to insufficient research in the elderly, caution is needed when prescribing. Overdose requires monitoring for 24 hours, and most side effects are temporary and reversible. Keep this medication out of reach of children and store it away from direct sunlight and moisture.
