Precautions

This drug consists of sitagliptin and metformin, and is used in the treatment of diabetes. Firstly, this drug can cause serious hypersensitivity reactions, although rare, so if allergic symptoms or skin abnormalities occur within 3 months after administration, immediate discontinuation and consultation with a physician are required. Additionally, pancreatitis, which is an inflammation of the pancreas, may occur, so if symptoms of pancreatitis such as abdominal pain, nausea, or vomiting appear, the use of this drug should be discontinued immediately and medical attention sought. The metformin contained in this drug may cause a rare complication called lactic acidosis, which can be dangerous due to excessive accumulation of lactic acid in the body, hence it should not be used in cases of severe dehydration, cardiovascular disease, liver disease, or acute infection. Patients with moderately compromised kidney function or with acute heart failure, acute myocardial infarction, or those undergoing iodine contrast imaging procedures should not be administered this drug, and it should be avoided in pregnant or breastfeeding women. While taking this medication, renal function should be monitored regularly to ensure there are no abnormalities, and especially for elderly patients or those with compromised renal function, caution is necessary. Since this medication lowers blood sugar, if meals are skipped or physical activity increases suddenly, hypoglycemia might occur, so attention to symptoms is necessary. When used with other hypoglycemic agents or insulin, the risk of hypoglycemia may increase, and dosage adjustments may be needed. Alcohol consumption can increase the risk of lactic acidosis, so excessive drinking should be avoided during the course of this medication. In case of surgery, this medication should be discontinued 48 hours prior to the procedure and can be resumed after oral intake and normal recovery of renal function post-surgery. Also, while taking this medication, it is recommended to regularly check blood tests and vitamin B12 levels to ensure there are no deficiencies. Lastly, safety and efficacy have not been established for children, and it is not recommended for individuals under 18 years of age. Adhering closely to these precautions will help this medication contribute to diabetes management.
