This medication is used as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. In particular, the combination of empagliflozin and metformin aids in blood sugar management. Moreover, it is important to note the heightened risk of cardiovascular events in patients with cardiovascular disease who do not achieve adequate blood sugar control. This medication is effective in improving diabetes management and blood sugar control.

How to take

Adult dose

The recommended dosage is 1 tablet once, twice daily. In cases where glycemic control is inadequate with monotherapy of metformin or in combination with other medications such as insulin, empagliflozin 5 mg is administered twice daily (total daily dosage of 10 mg) along with the patient's existing metformin dosage. In instances of good tolerance and where additional blood sugar control is necessary, the dosage may be increased up to empagliflozin 25 mg (total daily dosage). For patients with renal impairment, dosage adjustments may be necessary depending on renal function.

Precautions

This medication is used for the treatment of diabetes, utilizing two components to achieve blood sugar control. Caution is advised as a rare side effect known as severe lactic acidosis may occur while using this medication. Lactic acidosis can lead to an excessive accumulation of lactate in the body, posing life-threatening risks. Additionally, concomitant administration with insulin or sulfonylureas may increase the risk of hypoglycemia or dangerously low blood sugar levels, necessitating special care. This medication should not be used in patients with moderate to severe renal impairment or those with certain acute conditions, such as dehydration, severe infections, or heart function failure. It is advised to temporarily discontinue this medication before and after tests involving radiocontrast iodine agents and resume only after renal function has been confirmed as normal. Pregnant or breastfeeding women should refrain from using this medication and, especially, women with potential for pregnancy should avoid it. During treatment, it is fundamental to regularly monitor renal function to ensure the drug is used safely. Reported side effects include hypoglycemia, urinary tract infections, genital infections, increased urination, gastrointestinal disturbances (nausea, vomiting, diarrhea), abnormal liver function, etc. In particular, this medication may cause fluid loss; therefore, elderly patients or those at risk of dehydration should be particularly cautious while using it. When taking this medication alongside other drugs, possible interactions might occur, so all medications being used should be disclosed to healthcare providers. In case of an overdose, blood dialysis may be necessary, confirming the importance of adhering strictly to the prescribed dosage. This medication is not recommended for children, and dosage adjustment may be needed in elderly patients based on renal function. Lastly, immediate consultation with healthcare providers is essential if symptoms such as fever, genital discomfort or swelling, or skin rashes occur during treatment.
