Treatment of essential hypertension and angina pectoris
This medication is used to treat angina and essential hypertension from mild to moderate levels. Angina is a condition that occurs due to insufficient blood supply to the heart, and essential hypertension refers to elevated blood pressure without a specific cause. This medicine helps improve symptoms by dilating blood vessels to facilitate blood flow and stabilize blood pressure. Thus, it is effective in treating angina and essential hypertension.
1. Adults: The initial dose is 180㎎ of diltiazem hydrochloride orally once daily. The dose may be increased at 2-4 week intervals according to symptoms. The usual maintenance dose is 240-360mg once daily. The maximum daily dose is 360mg. 2. Elderly patients and patients with hepatic or renal impairment: The initial dose is 120㎎ once daily. Measure heart rate during treatment; do not increase the dose if it falls to 50 beats or fewer.
This medication should not be administered to patients with severe decompensated heart failure or cardiac conduction disturbances. Additionally, it should not be used in patients with extremely low blood pressure, allergy to this drug, or in pregnant or potentially pregnant women. Caution should be exercised in patients with severely impaired liver or kidney function, mild cardiac conduction disturbances, or those with abnormal gastrointestinal transit time due to chronic diarrhea. When used in combination with beta-blockers and others, careful monitoring is required due to the risk of bradycardia. Side effects may include edema, dizziness, vasodilation, pharyngitis, diarrhea, palpitations, rash, and various other symptoms. Rarely, severe adverse reactions such as arrhythmias, liver dysfunction, and skin rashes have been reported. Sudden discontinuation may exacerbate symptoms; hence, dosage should be tapered gradually as per the physician's instruction, and regular cardiac monitoring is necessary due to potential ECG anomalies. Furthermore, since this drug is metabolized in the liver and excreted by the kidneys and bile, dosage adjustment and monitoring are essential in patients with liver or kidney dysfunction. Dizziness or fatigue may occur during driving or operating hazardous machinery, requiring caution. Patients with bronchial hyperreactivity, such as asthma, should use this medication carefully as respiratory distress may worsen. This drug interacts with multiple medications, especially when used with beta-blockers, digitalis, some antibiotics, antidepressants, antifungals, and statins, leading to changes in drug concentration and risks of side effects; thus, consultation with a doctor or pharmacist is imperative. Pregnant or nursing women should ideally avoid this drug, and if absolutely necessary, nursing should be discontinued. The safety and efficacy in pediatric patients have not been established. The elderly often have impaired liver, kidney, or heart function and are more likely to be on multiple medications, so treatment should start at lower doses and be carried out cautiously. Overdose may result in severe hypotension, bradycardia, and conduction disturbances, requiring immediate medical attention, and may necessitate emergency measures like inducing vomiting or administering activated charcoal. This drug comes in capsule form and should be swallowed whole without opening or chewing; it should be stored out of reach of children and kept in its original container.
