This medication is used for the treatment of adult patients with type 2 diabetes whose blood sugar control is not sufficient with diet and exercise alone, particularly for overweight diabetes patients. It can be used alone or in combination with other oral hypoglycemic agents or insulin to effectively control blood sugar levels. This helps improve the patient's blood sugar levels and aids in the prevention of diabetes-related complications.

How to take

Adult dose

Administer Metformin hydrochloride extended-release 500 mg once daily with the evening meal. Measure blood sugar levels after one week to adjust dosage and gradually increase by 500 mg each week up to a maximum of 2000 mg per day with the evening meal. Consider administering 1000 mg twice daily with meals if blood sugar control is not achieved with a maximum single dose of 2000 mg per day. For patients with renal impairment, the starting dose is adjusted.

Precautions

This medication can occasionally cause a rare but serious condition known as lactic acidosis. Lactic acidosis is a disease resulting from abnormally high levels of lactic acid in the blood, and in severe cases, it can be life-threatening. Caution is necessary when using this drug in conjunction with other diabetes medications such as insulin or sulfonylureas, as it may lead to hypoglycemia. Patients with moderate or severe renal impairment, dehydration, severe infections, acute myocardial infarction, or other acute illnesses should not use this drug. Additionally, patients with acute heart failure or those undergoing specific tests using iodine contrast agents should also avoid treatment with this medication. It is not to be used in patients with an allergy to any component of this drug or those with type 1 diabetes or diabetic ketoacidosis. Patients with severe infections or major traumas should temporarily discontinue this medication, as should those undergoing surgery, for a certain period around the procedure. Caution is also advised for patients with impaired liver function, excessive alcohol intake, dehydration, vomiting, or gastrointestinal disorders. Kidney function should be regularly monitored during treatment with this medication, and it should be discontinued if kidney function deteriorates. Patients with irregular or insufficient food intake, engaging in vigorous exercise, or taking other medications should use this drug carefully due to the increased risk of hypoglycemia or lactic acidosis. Gastrointestinal disturbances such as nausea, vomiting, diarrhea, and abdominal pain are common when starting this medication and usually resolve over time. Vitamin B12 deficiency may occur, so it is recommended to regularly check serum vitamin B12 levels. During pregnancy, consultation with a doctor is necessary as it may affect the fetus if planning for pregnancy or currently pregnant, and breastfeeding is not recommended on this medication. Elderly patients are likely to have decreased kidney function, so kidney function should be checked before and regularly monitored during treatment. In case of overdose, hypoglycemia may be uncommon, but there is a risk of lactic acidosis, requiring immediate medical attention for appropriate treatment. This drug is not recommended for individuals under 17 years old as safety and efficacy have not been established in this age group. During treatment, alcohol consumption should be avoided, and interactions with other medications should be discussed with a physician or pharmacist. The tablets should be swallowed whole and not chewed or crushed, and the tablet coating may be excreted in the stool, which is normal. Finally, this medication should be kept out of reach of children and not stored in other containers.
