Treatment of hypertension (secondary, resistant)
This medication is used for the treatment of hypertension caused by secondary conditions or target organ damage. It is also effective in resistant hypertension that does not respond even when combined with diuretics and two types of antihypertensive agents. It helps improve these hypertensive states and aids in blood pressure control.
Adults (aged 12 years and older): An initial dose of 5mg of Minoxidil may be taken orally once daily, with a maintenance dose of 10-40mg administered in one or two divided doses per day. A maximum of 100mg per day may be administered. Dosing frequency: If the reduction of the effective pressure is less than 30mmHg, it can be taken once a day, and if it is 30mmHg or more, it should be taken in two divided doses. Dosage change: Adjustments should be made carefully based on the patient’s response.
Children (aged 12 years and below): An initial dose of 0.2mg per kg body weight given once daily may be administered, and the dose can be increased by 50-100% until blood pressure is optimally adjusted. The maintenance dose is 0
This medication should not be used in patients with certain conditions. For example, patients with pheochromocytoma should not be administered this medication because it can lower blood pressure while stimulating the secretion of specific hormones from the tumor. Patients with pulmonary hypertension due to mitral stenosis, patients with acute myocardial infarction, and patients who have had an allergic reaction to this medication should also avoid it. Caution should be exercised when administering this medication to patients with chronic congestive heart failure, severe renal failure, and angina pectoris, and their condition should be monitored closely according to the physician's instructions. When taking this medication, excessive hair growth or thickening may occur, primarily appearing on the face and body, which will gradually return to normal upon discontinuation of the medication. Skin rashes or allergic reactions may also occur, so care is necessary. Blood tests may show a decrease in platelet or white blood cell counts, and liver function tests may change. Changes in cardiac electrical signals may be observed in electrocardiogram tests; however, most recover with continued treatment and do not cause significant issues with heart function. Rarely, pericardial effusion can occur, especially in patients with poor kidney function, who need to exercise even more caution and may require further examinations if symptoms arise. During treatment with this medication, fluid retention leading to edema may occur, so it is usually used in combination with diuretics to manage fluid balance. Since the medication causes vasodilation, heart rate may increase; thus, it is often taken with other medications to prevent this symptom. Patients with reduced kidney function or those on dialysis should have their dosage adjusted, and it should only be used in stable status after myocardial infarction. Regular examinations, including urinalysis, kidney function tests, electrocardiograms, and chest X-rays, are necessary at the start of the treatment. This medication can interact with other drugs, so caution is needed, especially when taken with antihypertensive medications, antidepressants, or non-steroidal anti-inflammatory drugs. Safety during pregnancy has not been established, and it is recommended to avoid use during breastfeeding or to stop nursing if necessary. Experience with this drug in children, particularly neonates, is limited, and it should be administered carefully. In the event of overdose, hypotension may worsen, so immediate medical attention is required to receive appropriate treatment, and certain medications or saline solutions can be used to control blood pressure.
Exfoliative dermatitis, vesicular lesions, skin and mucous membrane syndrome (Stevens-Johnson syndrome), pericardial effusion and thrombosis, fluid retention and congestive heart failure, reflex tachycardia and angina, deterioration of kidney function, unstable status after myocardial infarction, orthostatic hypotension, severe allergic reactions, severe edema or urination abnormalities.
Hypertrichosis, weight gain, peripheral edema, increased heart rate, transient increase in creatinine and BUN, gastrointestinal disturbances, rash, breast tenderness, gynecomastia, hemolytic anemia, pulmonary infiltration, antinuclear antibody (ANA), headache, intermittent claudication, frequent menstruation, nausea, vomiting, thrombocytopenia, leukopenia, decreases in hematocrit and hemoglobin, increased ALP, changes in electrocardiogram T-wave direction and amplitude, pericardial effusion (among non-dialysis patients)
