This medication is used to treat severe primary genital herpes, recurrent varicella-zoster virus infection, and herpes simplex encephalitis in patients with normal immune function. It is also effective in treating and preventing herpes simplex virus infections, as well as treating primary and recurrent varicella-zoster virus infections and herpes simplex encephalitis in patients with impaired immune function. It is used for the treatment of various herpes simplex and varicella-zoster related viral infections.

How it is given

Adult administration

The injection should be slowly infused over at least 1 hour. Herpes simplex infection: Administer 5mg per kg of body weight every 8 hours. Recurrent varicella-zoster virus infections: Administer 5mg per kg of body weight every 8 hours. Primary and recurrent varicella-zoster virus infections: Administer 10mg per kg of body weight every 8 hours. Herpes simplex encephalitis: Administer 10mg per kg of body weight every 8 hours.

Child administration

Precautions

This medication should not be used in patients who have had an allergic reaction to this medication, its components, or valacyclovir. Patients with impaired renal function should use with caution as neuropsychiatric adverse reactions may occur. Patients with poor liver function should use this medication carefully as it may worsen liver function. It should be used with special caution in infants, children, and the elderly. While using this medication, a severe allergic reaction known as anaphylactic shock may rarely occur, so if symptoms such as hypotension, dyspnea, or chest tightness appear, stop using the medication immediately and seek appropriate treatment. Hypersensitivity reactions such as rash, fever, vesicles, urticaria may occur; if these symptoms arise, stop the medication and consult a doctor. Skin-related side effects may include hair loss, erythema multiforme, severe skin conditions such as Stevens-Johnson syndrome or toxic epidermal necrolysis. Neuropsychiatric side effects may include altered consciousness, hallucinations, seizures, anxiety, dizziness, confusion, numbness, tremors, drowsiness, headaches, and fatigue. Kidney problems may lead to acute renal failure, hematuria, proteinuria, and if any issues occur, the dosage should be adjusted or administration should be stopped. Liver effects can lead to hepatitis, jaundice, and elevated liver function test values; if any symptoms of liver dysfunction occur, discontinue the medication. Hematologic side effects can include bleeding, changes in white blood cell or platelet counts, and anemia. Gastrointestinal side effects may include gastritis, dyspepsia, nausea, vomiting, abdominal pain, diarrhea, and loss of appetite. Cardiovascular side effects may include chest pain, palpitations, arrhythmias, and changes in blood pressure. Muscle pain or joint pain may also occur. Urological effects may include dysuria or urinary obstruction. General side effects may include edema, syncope, muscle weakness, fever, headaches, pallor, and feeling cold. Other reported side effects include pneumonia, laryngitis, visual disturbances, taste disorders, hair loss, sweating, and electrolyte abnormalities. When administering the medication, it is effective to start as soon as possible, especially at high doses where adequate hydration is necessary to prevent dehydration and reduce the risk of renal toxicity. This medication may interact with other drugs, so caution should be taken when used concurrently with medications such as probenecid, cimetidine, mycophenolate mofetil, and theophylline. During pregnancy, this should only be used when absolutely necessary, as it may affect the fetus; during breastfeeding, it should be discontinued as the drug may be excreted in breast milk. In infants and children, it should be administered cautiously only when necessary, and in the elderly, dosage adjustment may be required due to increased drug concentration resulting from reduced renal function. In cases of overdose, symptoms may include agitation, coma, seizures, drowsiness, and renal function may worsen, therefore immediate medical advice should be sought and if necessary, treatment such as hemodialysis should be provided. This drug should be administered slowly through a drip infusion rather than by rapid intravenous injection. Care should be taken to prevent extravasation at the injection site. Before use, the solution should be well shaken, and if precipitation occurs upon dilution, it should not be used. Mixing with other drugs should be avoided as it is alkaline, and changes in pH can easily lead to incompatibility.

Anaphylactic shock, anaphylactoid reactions, hypotension, dyspnea, chest tightness, urticaria, severe swelling, severe rash with high fever, blistering, skin peeling, mucosal pain in the mouth/eyes, mucocutaneous syndrome (Stevens-Johnson syndrome), toxic epidermal necrolysis (Lyell syndrome), altered consciousness, seizures, limb numbness, drowsiness, sudden and severe headaches, profound malaise, jaundice, acute renal failure, and hypertension.
