Treatment of severe liver dysfunction and brain function impairment (hepatic coma)
This medication is used for severe liver dysfunction, with or without brain function impairment, namely for patients with liver failure. It is also effective in treating hepatic coma. It helps improve liver function and alleviate symptoms caused by brain function impairment. This drug is utilized as an essential treatment for patients with severe liver disease. Therefore, this medication is used for the treatment of severe liver dysfunction and hepatic coma.
Total amino acids at 1.5 g per day per kg of body weight as the maximum dose (equivalent to 2100 mL per day for a weight of 70 kg) via slow intravenous infusion. The administration rate should be 1.7 to 2.1 mL per kg of body weight over 60 minutes (for 70 kg, this translates to 40 to 50 drops per minute).
This medication should only be used according to a doctor's instructions. It should not be used in patients with severely impaired kidney function or high nitrogen levels in the blood. Patients with amino acid metabolism issues other than liver dysfunction should not be given this medication. Administration should also be avoided in patients with metabolic disorders that impair nitrogen utilization. Patients with severely weakened cardiac function due to wasting heart disease and patients with pulmonary edema due to fluid buildup in the lungs should not use this medication. It should not be given to patients with oliguria or anuria, where urinary output is minimal or nonexistent. Formulations containing sodium and chloride ions should be used cautiously in patients with high sodium or chloride levels in the blood. In cases where magnesium ions are included, it should not be administered to patients with heart conduction disorders like heart block or severe myasthenia gravis. Administration should be avoided in patients who are overly hydrated. Patients with a history of myocardial infarction or those who have had one previously should be monitored closely. Caution is needed when administering to patients with severe acidosis or congestive heart failure. Care should also be taken with patients who have excessive sodium in the body causing edema and those with abnormal potassium levels. For patients with impaired kidney function, dosage and administration rates should be adjusted carefully. Rarely, hypersensitivity reactions like skin rashes may occur; in such cases, administration should be stopped immediately. Occasionally, gastrointestinal symptoms such as nausea or vomiting may occur. Cardiac symptoms may include chest discomfort, rapid heartbeat, and elevated blood pressure. If administered too quickly and in excess, metabolic acidosis can occur due to excess acidity in the body. There may be occurrences of chills, fever, headache, difficulty breathing, and in severe cases, respiratory arrest or shock. Formulations containing magnesium ions may result in peripheral vasodilation. Metabolic disorders can lead to complications such as acidosis, hypophosphatemia, alkalosis, hyperglycemia, diabetes, osmotic diuresis, and dehydration. Patients with decreased liver or kidney function may experience elevated nitrogen levels in the blood. Patients with severe liver dysfunction are at risk for amino acid imbalance, consciousness disorders, and coma. Due to variations in treatment efficacy, periodic monitoring of blood ammonia levels is necessary. When given in large doses, careful attention to electrolyte balance is required, especially when used in conjunction with other electrolyte-containing medications. Caution is needed to prevent metabolic alkalosis from overdosing. Patients with a history of kidney disease, lung disease, or heart disease should be administered cautiously to avoid excessive fluid supply to the body. Diabetic patients should frequently monitor their blood sugar levels during administration. Patients with hyponatremia or liver/kidney dysfunction should individually manage dosage and fluid balance along with electrolyte levels in the blood. When co-administered with certain antibiotics like tetracycline, the protein-sparing effect may be reduced. If magnesium ions are present, caution should be taken when giving calcium injections concurrently as magnesium may precipitate. The safety of this medication in pregnant women has not been sufficiently established, and it should only be administered when the benefits outweigh the risks. When administering to children, the dosage should be adjusted to lower concentrations since liver function may be immature. Infants, in particular, may have an impaired ability to synthesize urea, leading to elevated blood ammonia levels; therefore, frequent blood testing is necessary. Elderly patients generally have diminished body functions, so dosage should be reduced, and infusion rates should be slow. This medication contains electrolytes; thus, careful attention to electrolyte balance is essential when administered in large doses or with other electrolyte solutions. Administration should be done slowly intravenously; too rapid administration may cause chills or vomiting, and excess amino acids may be excreted via the kidneys. In cold weather, it is advisable to warm the solution to body temperature before use. As there is a risk of infection during administration, utmost care should be taken to maintain sterile conditions when preparing the medication and sterilizing injection equipment. If the solution appears cloudy, discolored, leaks, or has droplets, it should not be used. The measuring scale of the container should be checked accurately for use, and during storage, it should be kept well-packaged unopened.
Not everyone experiences these, and they can vary from person to person.
This medication should be stored in a sealed container that prevents air entry. Additionally, it is advisable to keep it at room temperature between 1 degree and 30 degrees Celsius. Proper storage will help maintain the efficacy of the medication.
