Treatment of herpes zoster; treatment of genital herpes and suppression of recurrence
This medication is used for the treatment of herpes zoster virus infections and genital herpes infections. It is also effective in suppressing the recurrence of recurrent genital herpes. Thus, it helps alleviate the symptoms of these viral diseases and prevent recurrences.
1. Treatment of herpes zoster Adults: Administer 250 mg of famciclovir orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of first-episode genital herpes Adults: Administer 250 mg of this medicine orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes Adults: Administer 125 mg of this medicine orally 2 times daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medicine orally 2 times daily. Periodically discontinue treatment every 6~12 months to assess whether the natural course of the disease has changed. 5. Patients with renal impairment: Since the creatinine clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be undertaken with caution. The dosage for these patients is as follows. 1) Treatment of herpes zoster and first-episode genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose, 2 times daily 10~29: 250 mg per dose, once daily 2) Treatment of acute recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 125 mg per dose, 2 times daily 10~29: 125 mg per dose, once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose, 2 times daily 10~29: 125 mg per dose, 2 times daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. A 4-hour hemodialysis session reduces the plasma concentration of this medicine by approximately 75%; therefore, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is required for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.
This medication should not be used in patients who have had an allergic reaction to this drug, its components, or the active ingredient, penciclovir. Additionally, this medication contains lactose, so patients with certain genetic conditions that disable lactose digestion should not take it. Patients with impaired kidney function should take special care when using this medication, and dosage adjustments may be necessary. While taking this medication, side effects such as headaches, dizziness, nausea, vomiting, and skin rashes may occur, so if any unusual symptoms arise, consulting a doctor is advised. In particular, elderly patients may experience central nervous system symptoms such as drowsiness or confusion more frequently, so caution is required. This medication does not completely cure genital herpes, so it is important to avoid sexual contact and maintain safe sex practices even if symptoms are present during treatment. If dizziness or drowsiness occurs, it is advisable to avoid driving or operating machinery. Patients with liver dysfunction generally do not need dosage adjustments, but caution is necessary due to a lack of adequate studies in patients with severe liver disease. This medication tends to have minimal drug interactions, but when taken with certain medications, blood concentration changes or toxicity potential should be monitored. Pregnant or nursing women should consult a doctor before using this medication, as its safety has not been established. It is not recommended to use this medication in children under the age of 18, as its safety and efficacy have not been sufficiently verified. Elderly patients generally do not need dosage adjustments, but if kidney function is impaired, dosage adjustments should be made. In cases of overdose, appropriate treatment along with hemodialysis may be helpful, as the active ingredient can be significantly removed through hemodialysis. This medication should be stored in a tightly closed container to prevent air or moisture from entering, and kept in a cool, dry place below 30 degrees Celsius, out of reach of children, and not transferred to other containers.
