Summary

Main use
Treatment of obesity (weight loss and maintenance); reduction of the risk of weight regain
Who takes it
Adults · Children
Adult dose
The recommended dose of this medicine is 120 mg of orlistat orally 3 times daily, taken with a meal containing fat or within 1 hour after a meal. Patients should continue a nutritionally balanced, mildly low-calorie diet in which approximately 30 % of calories are derived from fat. Daily intake of fat, carbohydrate, and protein should be distributed evenly among the three meals. This medicine may be omitted if a meal is skipped or contains no fat. Because this medicine may reduce the absorption of some fat-soluble vitamins and beta-carotene, patients are advised to take a multivitamin containing fat-soluble vitamins to ensure adequate nutrition. The vitamin supplement should be taken at least 2 hours before this medicine or at least 2 hours after it, such as at bedtime. Taking more than 120 mg of this medicine 3 times daily does not increase its effectiveness. Based on measurements of fecal fat, the effect of this medicine appears approximately 24 ∼ 48 hours after taking it. After discontinuation, fecal fat content generally returns to pretreatment levels within 48 ∼ 72 hours. The efficacy and safety of this medicine for periods exceeding 4 years have not been evaluated. Studies have not been conducted in patients with hepatic and/or renal impairment or in elderly patients.
Child dose
Studies have not been conducted in children under 12 years of age.

Precautions

This medication should not be used in patients with chronic malabsorption syndrome or those with biliary secretion stopped. Patients who are allergic to this medication or its components should also avoid use. It is advisable not to use this medication in pregnant women or those who may become pregnant, as well as in breastfeeding women. Liver damage has been reported rarely; therefore, if symptoms indicating liver dysfunction occur, patients should immediately inform their doctor and discontinue use of this medication. Caution should also be taken in patients with hyperoxaluria or calcium oxalate kidney stones. Use of this medication may frequently cause gastrointestinal symptoms such as abdominal pain, gas, constipation, soft or oily stools, and increased bowel movements. These symptoms are usually mild and temporary and tend to decrease over time. However, some patients may experience symptoms lasting longer than 6 months. Symptoms severe enough to necessitate discontinuation of the medication may occur, so patients should consult a doctor if symptoms are severe. Taking this medication with a high-fat meal may worsen gastrointestinal side effects; therefore, it is important to properly moderate fat intake, distributing it evenly across three meals per day. Additionally, because this medication may interfere with the absorption of fat-soluble vitamins and beta-carotene, it is recommended to take vitamin supplements as well. These supplements should be taken at least 2 hours before or after taking this medication. Signs of liver dysfunction include decreased appetite, itching, jaundice, changes in urine color, changes in stool color, and right upper abdominal pain; such symptoms should be reported to a doctor immediately. Diabetic patients may experience improved blood sugar control with this medication, which may require dosage adjustments of blood sugar-lowering agents. Additionally, excessive weight loss may increase the risk of gallstones. This medication may interact with specific drugs; therefore, patients taking cyclosporine, anticoagulants, antiepileptics, antidepressants, antipsychotics, or antiretroviral drugs should consult their doctor. Pregnant or breastfeeding women should not take this medication and it is not recommended for children under 12 years of age. In the case of overdose, observation for 24 hours is required, and the effect of this medication recovers quickly. When storing this medication, keep it out of reach of children and avoid exposure to light and moisture by storing in its original packaging.
