Summary

Main use
Prevention of thrombotic cardiovascular events (such as myocardial infarction and stroke) and stroke
Who takes it
Adults · Children
Adult dose
This medicine should not be used in combination with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer a loading dose of 180mg, followed by 90mg twice daily. Treatment for 12 months is recommended unless discontinuation of this medicine is clinically necessary. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. In patients with acute coronary syndrome who are at increased risk of bleeding and have undergone percutaneous coronary intervention (PCI), monotherapy with this medicine (discontinuation of aspirin) may be considered after 3 months of treatment with this medicine and aspirin. Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Patients who received 90mg of this medicine or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with this medicine at 60mg. Treatment with this medicine may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuing a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. For patients with acute coronary syndrome, refer to the corresponding dosage and administration. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer a loading dose of 180mg, followed by 90mg twice daily for 30 days. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended in patients with acute ischemic stroke or high-risk transient ischemic attack. This medicine is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this medicine into a fine powder, disperse it in half a glass of water, and drink it immediately. Rinse the glass with another half glass of water and drink it. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the tube with water after administration. When switching from another antiplatelet agent to this medicine, start this medicine 24 hours after the last dose of the previous medicine. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is necessary in patients with renal impairment. No dose adjustment is necessary in patients with mild hepatic impairment. This medicine has not been studied in patients with severe hepatic impairment, and experience in patients with moderate hepatic impairment is limited.
Child dose
The safety and efficacy of this medicine have not been established in pediatric patients younger than 18 years.

This medication helps reduce the risk of thrombotic cardiovascular events, such as myocardial infarction or stroke, when used in conjunction with aspirin. It is particularly effective in patients with acute coronary syndrome or high-risk patients who have previously experienced myocardial infarction. Additionally, it reduces the occurrence of myocardial infarction and stroke in patients with type 2 diabetes who have undergone coronary interventions due to coronary artery disease. It is used in combination with aspirin to reduce the incidence of stroke in adult patients with acute ischemic stroke or high-risk transient ischemic attack. This improves cardiovascular health and prevents the recurrence of critical events.

Precautions

This medication is designed to prevent the blood from clotting, thus preventing thrombosis. However, it can increase the risk of bleeding; therefore, it should not be used in individuals with pathological active bleeding or a history of intracranial hemorrhage. When undergoing surgery, it is advised to stop this medication at least 5 days prior to reduce the risk of bleeding. Patients who have recently undergone cardiac procedures or surgeries should be cautious about signs of hypotension, which may indicate bleeding, and it is advisable to manage bleeding without discontinuing the medication if possible. Those with allergies to any of the components of this medication, severe bleeding, severe liver dysfunction, or those taking certain strong drugs should not take this medication. Patients at high risk of bleeding should use this medication cautiously, particularly if they have had recent trauma or surgery, history of coagulation issues, or gastrointestinal bleeding. Additionally, caution should be taken when combined with other medications that may increase bleeding risk. Patients scheduled for surgery should inform their doctor about taking this medication and discontinue it about 5 days prior to surgery. Patients at risk of bradycardia (slow heart rate) or dyspnea should also be cautious when taking this medication. Patients with asthma or chronic obstructive pulmonary disease (COPD) may experience worsened dyspnea while on this medication and should be monitored accordingly. Pregnant or nursing women should take caution when considering this medication, and those planning to become pregnant should avoid conceiving during the course of treatment. Safety and effectiveness in patients under 18 years of age have not been established and is not recommended. Overdose may increase the risk of bleeding, and since there is no specific antidote, immediate medical assistance is required. This medication may interact with other drugs, so patients should notify healthcare providers of any other medications being taken. Caution should be exercised when driving or operating machinery as dizziness or confusion may occur. During treatment with this medication, patients should immediately consult a doctor if symptoms of bleeding, dyspnea, or any irregularities in heart rhythm occur.
