Prevention of thrombotic cardiovascular events (myocardial infarction, stroke, etc.)
This medication is effective in reducing the risk of cardiovascular diseases when used in conjunction with aspirin. It decreases the incidence of thrombotic cardiovascular events in patients with acute coronary syndrome or those with a history of myocardial infarction. Additionally, it helps prevent myocardial infarction or stroke in patients with coronary artery disease who have type 2 diabetes and have undergone coronary intervention. It is also used to reduce the incidence of stroke in patients with acute ischemic stroke or high-risk transient ischemic attacks. Thus, it serves as an effective treatment option to reduce the occurrence of cardiovascular events such as myocardial infarction and stroke in patients at high risk of cardiovascular complications.
This drug should not be used concomitantly with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer an initial dose of 180mg, followed by 90mg twice daily. Treatment for 12 months is recommended unless discontinuation of this drug is clinically necessary. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. In patients with acute coronary syndrome at increased risk of bleeding who have undergone percutaneous coronary intervention (PCI), monotherapy with this drug (discontinuation of aspirin) may be considered after 3 months of concomitant treatment with this drug and aspirin. (See section 3. Patients Requiring Cautious Administration.) Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Patients who took 90mg of this drug or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with 60mg of this drug. Treatment with this drug may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuing a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). The safety and efficacy of this drug were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. For patients with acute coronary syndrome, refer to the corresponding dosage and administration. Data on the safety and efficacy of this drug when administered for more than 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer an initial dose of 180mg, followed by 90mg twice daily for 30 days. Patients taking this drug should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. Administration of an initial loading dose of aspirin (300-325mg) is recommended in patients with acute ischemic stroke or high-risk transient ischemic attack. This drug is administered orally and may be taken without regard to meals. Patients who have difficulty swallowing tablets should crush this drug into a fine powder, disperse it in half a glass of water, and drink it immediately; rinse the glass with another half glass of water and drink it. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the nasogastric tube with water after administration. When switching from another antiplatelet agent to this drug, start treatment with this drug 24 hours after the last dose of the previous medication. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dosage adjustment is required in patients with renal impairment. No dosage adjustment is required in patients with mild hepatic impairment. This drug has not been studied in patients with severe hepatic impairment, and there is limited experience with its use in patients with moderate hepatic impairment.
Safety and efficacy have not been established in pediatric patients younger than 18 years (see the “Other” section).
This medication prevents blood from clotting, thereby preventing the formation of thrombosis. However, there is a risk of bleeding, and it should not be used in patients with a history of bleeding or currently bleeding. If scheduled for surgery, the medication should be discontinued at least 5 days prior. If bleeding symptoms occur, contact a doctor immediately, and it's advisable to continue the medication while treating the bleeding. It should not be administered to patients allergic to its components, those with severe liver dysfunction, or those taking certain potent medications together. Use with caution in patients at high risk of bleeding or those taking other medications that may cause bleeding. Particularly, bleeding risk should be carefully managed in patients undergoing heart procedures. This medication may cause bradycardia, so caution is needed for patients with bradycardia-related conditions. Patients with asthma or chronic obstructive pulmonary disease should also be especially cautious as respiratory distress may occur. Women who are pregnant or breastfeeding should avoid using this medication or consult a doctor. Safety and efficacy have not been established for children. In case of overdose, the risk of bleeding increases, and appropriate treatment should be sought immediately. This medication may interact with multiple other medications, so inform your doctor or pharmacist about any other medications you are taking. Precautions are necessary as dizziness or confusion may occur while driving or operating machinery. Regular check-ups with a doctor during the use of this medication are required to monitor for bleeding or side effects.
