Diabetic patients may need to adjust their blood sugar-lowering medication dosages due to weight gain. If symptoms of hypersensitivity, such as angioedema, occur, discontinue the medication immediately. Elderly patients should be cautious during initial dosing due to increased fall risk from dizziness and drowsiness. Vision changes or blurred vision may occur, but these symptoms may improve upon discontinuation of the medication. Dose adjustment may be necessary for patients with renal impairment, and there have been reports of improved renal function after discontinuation of the medication. Patients with heart disease should use this medication cautiously considering the risk of congestive heart failure. Caution is necessary when used in combination with opioid analgesics due to increased risk of central nervous system depression. Patients with respiratory or neurological diseases or the elderly are at heightened risk for severe respiratory depression and should have their doses adjusted. In early pregnancy, caution is advised due to the risk of fetal malformations, and effective contraception should be used in women of childbearing age.
