This medicine is used for the treatment of essential hypertension and is also effective for heart failure patients who are not tolerant to ACE inhibitors (particularly NYHA class II to IV). Furthermore, it helps reduce the risk of mortality in patients with impaired left ventricular function after myocardial infarction. It is used in managing various cardiovascular diseases, thereby improving the patient's health.

How to take

Adult dose

Essential hypertension: The recommended dose is 80 mg orally once daily, which can be increased to 160 mg once daily and a maximum of 320 mg if necessary. Elderly patients (75 years or older) should start at 40 mg orally once daily. For patients with renal impairment with a creatinine clearance rate of 10-20 mL/min, the starting dose is 40 mg once daily; it should not be administered in patients with less than 10 mL/min clearance or in dialysis patients. This medication may cause serious harm to the fetus during pregnancy, especially in the second and third trimesters, so it should be discontinued immediately upon confirming pregnancy. People who are allergic to this medication or its components, pregnant or potentially pregnant women, and breastfeeding women should not take this medication. Patients with severely impaired liver function, bile flow problems, severe renal impairment, and specific hereditary angioedema should also refrain from using this medication. Elderly patients, those with high potassium levels, mildly impaired liver function, and those with heart valve disorders or cardiovascular diseases should use this medication with caution. This medication may cause a sudden drop in blood pressure upon initial administration, potentially leading to dizziness or fainting; therefore, especially for patients undergoing dialysis or taking diuretics, it is crucial to start with a low dose and gradually increase. During medication use, blood pressure, renal function, and electrolyte levels should be monitored regularly for any abnormalities. Caution is needed when using this medication with potassium-sparing diuretics, potassium supplements, or drugs that can elevate potassium levels, as it may dangerously increase serum potassium levels. Additionally, concomitant use with non-steroidal anti-inflammatory drugs (NSAIDs) may diminish the blood pressure-lowering effect and adversely impact kidney function; therefore, careful monitoring is necessary in such cases. This medication is not recommended for breastfeeding women. If it must be used, breastfeeding should be discontinued. Overdose may lead to severe hypotension, decreased consciousness, or heart rhythm abnormalities, requiring immediate medical treatment. This medication is not recommended for children and adolescents, as safety and efficacy have not been established for this age group. Symptoms such as dizziness may occur while driving or operating machinery; caution is advised. Lastly, this medication should be kept out of reach of children, and it is advisable not to transfer it to another container.
