Treatment of obesity (weight loss and maintenance), reduction and prevention of the risk of weight regain
This drug is used in obese patients with a body mass index (BMI) of 30 kg/m² or greater, or a BMI of 27 kg/m² or greater with risk factors such as hypertension, diabetes, or dyslipidemia. It assists in weight loss or maintenance in conjunction with a low-calorie diet and helps to reduce the risk of weight regain. Thus, it effectively supports weight management and the treatment of obesity.
The recommended dose of this drug is orlistat 120 mg orally 3 times daily, taken with a fat-containing meal or within 1 hour after a meal. Patients should continue a nutritionally balanced, mildly hypocaloric diet in which about 30 % of calories are derived from fat. Daily intake of fat, carbohydrates, and protein should be distributed evenly among the three meals. This drug may be omitted if a meal is skipped or contains no fat. Since this drug may reduce the absorption of some fat-soluble vitamins and beta-carotene, patients are advised to take a multivitamin containing fat-soluble vitamins to ensure adequate nutrition. The vitamin supplement should be taken at least 2 hours before this drug or at least 2 hours after it, such as at bedtime. Taking more than 120 mg of this drug per dose, 3 times daily, does not increase its effectiveness. Measurements of fecal fat indicate that the effect of this drug appears 24 ∼ 48 hours after administration. After treatment is discontinued, fecal fat content generally returns to pretreatment levels within 48 ∼ 72 hours. The efficacy and safety of this drug for periods exceeding 4 years have not been evaluated. Studies have not been conducted in patients with hepatic and/or renal impairment or in elderly patients.
Children younger than 12 years: No studies have been conducted.
This drug should not be used in patients with chronic malabsorption syndrome or complete biliary obstruction, patients allergic to this drug or its components, or women who are pregnant, may become pregnant, or are breastfeeding. It should be administered with caution in patients with impaired liver function, and if symptoms of liver dysfunction appear, the physician should be notified immediately, and use of this drug should be discontinued. This drug may frequently cause gastrointestinal adverse reactions, mainly abdominal distension, flatulence, fatty stools, and increased bowel movements, most of which are mild and transient. There may be cases where the intake of the drug is discontinued due to adverse reactions during treatment, so severe symptoms should be discussed with a physician. While taking this drug, it is important to avoid high-fat meals and to distribute daily fat intake evenly across three meals. This drug can interfere with the absorption of fat-soluble vitamins (vitamins A, D, E, and K) and beta-carotene, so taking vitamin supplements is recommended. Rare cases of liver damage or hepatitis, and anaphylaxis have been reported; if symptoms of liver dysfunction appear during treatment, immediately stop use and undergo testing. This drug may affect blood sugar control in diabetic patients, potentially necessitating adjustments in diabetes medication dosages. Excessive weight loss can increase the risk of gallstones, so caution is required. When taken with anticoagulants (e.g., warfarin), blood coagulation status should be checked frequently, and when taken with cyclosporine, cyclosporine levels may decrease, so dosing intervals should be adjusted. This drug is not recommended for children under 12 years of age, and its safety and efficacy in the elderly have not been sufficiently established. In case of overdose, 24-hour observation is required, and adverse reactions usually recover quickly. This drug should be stored out of reach of children, and it should be kept away from direct sunlight and moisture.
Not everyone experiences these, and they can vary from person to person.
This drug is sensitive to air, light, and moisture, so it should be stored in a specially sealed container. Additionally, it is important to store it in a cool place below 25 degrees Celsius to maintain its efficacy.
