Treatment for hypercholesterolemia (primary, familial) and reduction in cardiovascular disease risk
This medicine is used in patients with primary hypercholesterolemia and mixed hyperlipidemia when diet and exercise are insufficient for control. It particularly helps lower LDL cholesterol and total cholesterol levels in patients with familial hypercholesterolemia as an adjunct to diet therapy, delaying the progression of atherosclerosis. Additionally, it can be safely used in pediatric and adolescent patients with high LDL cholesterol levels. Furthermore, it is also used as an adjunct in the treatment of primary dysbetalipoproteinemia. In patients at high risk for cardiovascular disease, it is effective in reducing the risk of stroke, myocardial infarction, and risks associated with vascular reconstruction procedures. Overall, this medicine is actively used in the treatment of various hyperlipidemias and patients at risk for cardiovascular diseases.
Regardless of meals, the initial dose of 5 mg is administered once daily at any time of the day. If necessary, the maintenance dose can be adjusted to 10 mg once daily. The dose can be increased up to 20 mg once daily at intervals of 4 weeks or more based on LDL cholesterol levels and patient response. For Asian patients and those susceptible to myopathy, the recommended initial dose is 5 mg; a dose of 40 mg is contraindicated.
For pediatric patients aged 10 to 17 years with heterozygous familial hypercholesterolemia, the dosage is 5 mg to 20 mg once daily. The dosage is adjusted according to individual treatment goals over a period of 4 weeks.
This medicine should not be administered to certain patients. Those who are allergic to this medicine or have liver function issues, those with muscle disease, those with severely impaired kidney function, and women who are pregnant or breastfeeding should not take this medicine. Additionally, as this medicine contains lactose, caution is needed for those who are lactose intolerant. Before taking this medicine, regular liver function tests and muscle-related tests should be performed, and those with conditions such as hypothyroidism or nephrotic syndrome need to be treated first. If symptoms like myalgia or muscle weakness appear, consult a doctor immediately, and if symptoms are severe, discontinue use. This medicine should be used cautiously in those who consume excessive alcohol or have a history of liver disease. Moreover, this medicine can interact with other drugs, so it is essential to inform the doctor of all medications currently being taken, especially that the risk of muscle damage may increase when taken with fibrate medications. This medicine can rarely cause severe skin reactions or liver damage; thus, if symptoms like rash or jaundice occur, stop taking the medicine immediately and inform a doctor. Women who could become pregnant should use adequate contraception, and this medicine should not be taken during pregnancy. There are no specific antidotes for overdose, so treatment should be based on symptoms, and liver function and muscle enzyme levels should be monitored. Dizziness may occur while driving or operating machinery, so caution is needed. Regular tests and management per doctor's instructions during the course of this medicine are essential.
