Acute coronary syndrome; improvement of atherosclerotic symptoms; prevention of thrombi in atrial fibrillation
This medication is used to improve atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, and peripheral artery disease. Additionally, it helps to reduce the risk of death, myocardial infarction, and stroke due to cardiovascular issues in patients with acute coronary syndrome, particularly those who have received drug therapy, percutaneous coronary intervention, or coronary artery bypass surgery for unstable angina or non-Q-wave myocardial infarction. Furthermore, in patients with atrial fibrillation who have vascular risk factors and are unsuitable for vitamin K antagonists and have a low risk of bleeding, it decreases the risk of stroke and other thromboembolic events. This drug is effective for the prevention and treatment of various cardiovascular and cerebrovascular diseases.
○ Adults 1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer 75 mg of clopidogrel orally once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), on the day treatment with this drug is initiated, start with a loading dose of 300 mg of this drug once daily, followed by a maintenance dose of 75 mg orally once daily. Aspirin 75 ~ 325 mg should be administered concomitantly with this drug once daily. 3. For patients with atrial fibrillation, administer 75 mg of this drug orally once daily. Aspirin 75 ~ 100 mg should be administered concomitantly with this drug once daily. ○ Patients with renal impairment : Treatment experience in patients with renal impairment is limited. ○ Patients with hepatic impairment : Treatment experience in patients with moderate hepatic disease and a bleeding diathesis is limited. This drug may be administered regardless of food intake.
This medication prevents platelets from aggregating, thereby preventing blood clotting. It is activated in the liver by a specific enzyme, CYP2C19; individuals with genetically reduced enzyme function may experience decreased drug efficacy. Therefore, it is recommended to consider alternative treatment methods or other medications in such cases. Furthermore, concomitant administration with other drugs that activate CYP2C19 may increase the active ingredient concentration of this medication, raising the risk of bleeding, so it is advisable not to use them together. Individuals with allergies to this medication or patients with bleeding, severe liver impairment, and nursing mothers should not take this medication. Moreover, those with certain genetic disorders such as lactose intolerance should also refrain from using this medication. Precautions should be taken when using this medication in patients with a high risk of bleeding, especially those with gastrointestinal or ocular bleeding lesions, bleeding disorders, or patients scheduled for surgery. It is recommended to discontinue the medication 5-7 days prior to surgery and to immediately inform healthcare professionals if bleeding symptoms occur. This medication may increase bleeding risk when used in conjunction with other drugs that increase bleeding risk. While taking this medication, side effects such as bleeding symptoms, skin rashes, dyspepsia, diarrhea, headache, and dizziness may occur. In particular, bleeding symptoms can be severe, so it is crucial to consult a doctor immediately if any abnormal symptoms arise. During pregnancy, the medication should only be used when necessary, considering the potential impact on the fetus, and may be paused prior to delivery to reduce bleeding risk. Nursing mothers must discuss with their doctor whether to continue breastfeeding or stop taking the medication, as it may be secreted into breast milk. This medication is not recommended for children, as its safety and efficacy have not been established. Elderly patients or those with impaired renal function may have an increased risk of bleeding, necessitating dosage adjustment and monitoring of their condition. Overdose can increase the risk of bleeding; in such cases, platelet transfusion may reverse the effects of the medication. This medication should be stored out of the reach of children, and it is advisable not to transfer it to other containers. There is a potential for drug interactions during medication use, so inform your doctor or pharmacist of any other medications you are taking.
