The adult dosing is determined based on individual drug effectiveness and tolerability, and it should not exceed the recommended maximum daily dose. Start with a low dose and administer 500 mg 2 to 3 times a day with meals. Increase the dose gradually by 500 mg per week, and if the total reaches 2000 mg, it is typically divided into two doses in the morning and evening. If exceeding 2000 mg, it should be taken three times daily with meals.

Precautions

This drug can cause severe lactic acidosis or hypoglycemia, so it must be used with great caution. It should not be administered to patients with moderate to severe renal impairment, dehydration, serious infections, cardiovascular problems, etc. Patients who have acute heart failure or who are receiving iodinated contrast agents for specific examinations should temporarily discontinue this drug and can resume it once kidney function has been restored. It is not for use in patients with allergies to this drug or similar drugs, type 1 diabetes, or diabetic ketoacidosis. It should be temporarily discontinued in cases of severe infections, serious trauma, or before and after surgery, and can be resumed once the patient is stable. Patients with liver dysfunction, pulmonary embolism, severe lung dysfunction, and excessive alcohol consumption should avoid using this drug. It should not be used in pregnant women with placental dysfunction or at risk for preeclampsia. Irregular meals, intense exercise, and the use of other medications can increase the risk of lactic acidosis and hypoglycemia, so careful administration is needed. Lactic acidosis is very rare but can be fatal; symptoms like fatigue, muscle pain, shortness of breath, and abdominal pain should prompt immediate notification to a doctor. Gastrointestinal symptoms (diarrhea, nausea, vomiting, etc.) are common but usually transient, and can be alleviated by taking the drug with meals or gradually increasing the dose. If skin rash or blood abnormalities occur, administration should be stopped immediately, and regular checks for vitamin B12 deficiency are necessary. Hypoglycemia is rare when used alone, but can occur when taken with other blood sugar-lowering agents or alcohol, so caution is advised. This drug is mainly excreted through the kidneys, so it should be administered while receiving regular kidney function checks and dosage adjustment or cessation is necessary if kidney function declines. Alcohol consumption can interfere with lactic metabolism, increasing the risk of lactic acidosis, so excessive drinking should be avoided. Studies have shown effects on the fetus during pregnancy, so administration should be avoided if there are specific risks and fetal growth should be carefully monitored. This drug is secreted into breast milk, so a discussion with a doctor is needed to decide whether to continue breastfeeding or stop drug treatment. Elderly patients often have decreased kidney function, so starting with a lower dose and regular monitoring of kidney function is recommended. Due to potential drug interactions, inform healthcare providers of any current medications, and closely monitor blood sugar changes. In the event of an overdose, blood dialysis may be necessary, so immediate medical attention is required. This drug does not cause weight gain and helps increase blood sugar control and reduce the risk of diabetes-related complications.
