Summary

Main use
Treatment of symptoms of Alzheimer-type dementia
Who takes it
Adults · Children
Adult dose
○ Adults: Administer donepezil hydrochloride 5 mg once daily at bedtime. If sleep disturbances such as abnormal dreams, nightmares, or insomnia occur, morning administration of this medicine may be considered. Since donepezil reaches steady-state concentrations 15 days after administration and the incidence of adverse reactions may be affected by the rate of dose escalation, administer the 5 mg dose for 4~6 weeks. After evaluating the clinical response during this period, the dose may be increased to 10 mg. When increasing the dose to 10 mg daily, administer with caution for gastrointestinal adverse reactions. If treatment with this medicine is discontinued, its effects gradually diminish, and no rebound effect occurs following abrupt discontinuation. ○ Women aged 85 years or older with low body weight: Close monitoring is required because adverse reactions occur more frequently. Elderly women with low body weight should not exceed 5 mg daily.
Child dose
○ Children: There is no experience with use in children.

Precautions

This medication contains the ingredient donepezil hydrochloride and is primarily used to improve symptoms of Alzheimer's disease. Firstly, this medication should not be used in patients with an allergy to donepezil hydrochloride or any ingredients contained in this medication, in pregnant women or women who may become pregnant, and in breastfeeding women. Caution is required as this medication could cause bradycardia or arrhythmia in patients with heart disease or conduction disorders. Patients with a history of peptic ulcer disease or currently taking non-steroidal anti-inflammatory drugs should use this medication with caution, as gastrointestinal symptoms may worsen. Patients with asthma or chronic obstructive pulmonary disease may face the risk of bronchoconstriction or increased mucus secretion, worsening their symptoms, so they should use caution as well. Additionally, patients with extrapyramidal disorders such as Parkinson's disease should be cautious as symptoms may worsen due to this medication. This medication contains a dye called Yellow No. 5, so patients who are allergic to this ingredient should also be cautious. When taking this medication, various serious adverse reactions such as severe arrhythmia, liver dysfunction, gastrointestinal bleeding, cerebrovascular disorders, and movement disorders may occur, so immediate consultation with healthcare professionals is needed if symptoms arise. Common symptoms include nausea, vomiting, diarrhea, loss of appetite, dizziness, and insomnia, which are mostly mild or transient. This medication is used for the treatment of dementia and is not approved for other forms of dementia or memory decline, so it should be used only after a precise diagnosis. Patients with heart disease or electrolyte abnormalities are at risk of arrhythmia and may require an electrocardiogram. Caution is needed while driving or operating machinery, as dizziness or fatigue may occur, especially during the initial stages of treatment or after dosage adjustments. It is recommended that pregnant or breastfeeding women do not use this medication, and if necessary, they should consult a healthcare professional before making a decision. This medication is primarily metabolized in the liver, so patients with compromised liver function may be affected by its efficacy or side effects. Interactions may occur when taken with other medications, particularly with heart-related drugs, antibiotics, antidepressants, antipsychotics, and anti-inflammatory analgesics, so caution is advised when co-administering. In case of overdose, severe vomiting, bradycardia, hypotension, respiratory depression, and seizures may occur, requiring immediate treatment, with atropine as an antidote. Finally, this medication should be kept out of reach of children, and it is safe not to change the container.
