This medication is used to supply amino acids non-orally to patients with severe liver dysfunction, either with or without brain function impairment, namely liver failure patients. It is particularly effective for the treatment of hepatic encephalopathy, improving the patient's condition and aiding in treatment.
Dosage and Administration
Adult dosage
Administer intravenously by dripping the maximum dose of 1.5 g of total amino acids per kg of body weight daily (corresponding to 2100 mL daily for a weight of 70 kg). The infusion rate should be at 1.7 to 2.1 mL per kg of body weight over 60 minutes (40 to 50 drops per minute for a weight of 70 kg).
Precautions
This medication must be used strictly according to a physician's instructions. It should not be used in patients with severely impaired kidney function or elevated nitrogen levels in the blood. It should not be administered to patients with abnormalities in amino acid metabolism in addition to liver dysfunction. This drug should not be used in patients with metabolic disorders affecting nitrogen utilization, congestive heart failure, or pulmonary edema. Use of this medication is contraindicated in patients with very little or no urine output. If sodium or chloride ions are present, it should not be given to patients with high levels of sodium and chloride in the blood. If magnesium ions are present, it should not be used in patients with heart conduction abnormalities or severe myasthenia gravis. This medication should not be given to patients concerned about excessive fluid supply and those with a history of myocardial infarction. Caution should be exercised in patients with severe acidosis, those with heart failure, and those with high sodium accumulation in the body leading to edema. Patients with high potassium levels or issues with potassium excretion, as well as patients with impaired kidney function, should be monitored closely. The use of this medication may rarely lead to hypersensitivity reactions such as skin rashes, and if such symptoms occur, administration should be stopped immediately. Gastrointestinal symptoms such as nausea or vomiting may occur. Cardiovascular symptoms such as chest discomfort, palpitations, rapid pulse, and increased blood pressure may also occur. Rapid large doses may pose a risk of acidosis. Serious reactions such as chills, fever, headache, difficulty breathing, shock, bronchospasm, and urinary tract spasms have also been reported. In cases where magnesium ions are included, peripheral vascular dilation may occur. Metabolic acidosis, hypophosphatemia, alkalosis, hyperglycemia, diabetes, dehydration, hypoglycemia, increased liver enzymes, vitamin imbalances, electrolyte disturbances, and pediatric hyperammonemia may occur. Patients with impaired liver or kidney function may have elevated BUN levels in the blood. Patients with severely impaired liver function are at risk of developing amino acid imbalances in the blood, metabolic alkalosis, systemic nitrogen increase, hyperammonemia, and consciousness disorders. Blood ammonia levels may vary according to treatment effectiveness, thus periodic monitoring is necessary. When large doses are administered or when used with electrolyte-containing solutions, careful management of electrolyte balance is required. Overdose may lead to metabolic alkalosis, especially with solutions containing acetate ions, they should be used with caution. In patients with a history of kidney disease, pulmonary disease, or heart disease, excessive fluid supply should be avoided, so careful administration is required. Diabetic patients should monitor their blood sugar levels frequently while using this medication. Patients with hyponatremia or liver or kidney dysfunction should carefully manage dosage adjustments and hydration and electrolyte status. The protein-sparing effect of this medication may be reduced when used with certain antibiotics. If magnesium ions are present, caution should be taken when used with calcium injections as the drugs may precipitate. Safety in pregnant women has not been established, and it should only be administered when the benefits of treatment outweigh the risks. In children, the concentration should be kept low as liver function may be immature, and blood ammonia levels should be checked frequently. In the elderly, due to decreased body function, the administration rate should be reduced and dosages should be lowered if necessary. This medication contains electrolytes, and care should be taken regarding electrolyte balance when given in large amounts or combined with other electrolyte solutions. Infusing too quickly may cause chills or vomiting, and amino acids may be excessively excreted through the kidneys, so it should be administered slowly. It should be used within 24 hours when stored in the refrigerator, and it should be ensured that the solution is clear and not abnormal before use. Compounding must be done in sterile conditions and injection equipment must be thoroughly sterilized to reduce the risk of infection. If the packaging is damaged or if there is discoloration, leakage, or crystallization in the solution, it should not be used.
