Treatment of herpes zoster and genital herpes; suppression of recurrence
This medication is used to treat herpes zoster virus infections and genital herpes infections, especially to suppress recurrent genital herpes. It can help relieve symptoms of the viral infection and prevent recurrences.
1. Treatment of herpes zoster Adults: Administer 250 mg of famciclovir orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer 250 mg of this medicine orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer 125 mg of this medicine orally 2 times daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medicine orally 2 times daily. Discontinue treatment periodically every 6∼12 months to observe whether the natural course of the disease has changed. 5. Patients with renal impairment: Because the creatinine clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be undertaken with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30∼59: 250 mg per dose, 2 times daily 10∼29: 250 mg per dose, once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30∼59: 125 mg per dose, 2 times daily 10∼29: 125 mg per dose, once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) 30∼59: 250 mg per dose, 2 times daily 10∼29: 125 mg per dose, 2 times daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, it is preferable to administer the medicine every 48 hours, taking the dialysis interval into account. Four hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%; therefore, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary for patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, a clear recommended dose has not been established.
This medication should not be used in certain patients. First, patients who have had an allergic reaction to this medication or its components, including the active ingredient penciclovir, should not take it. In older individuals, symptoms such as drowsiness, confusion, and hallucinations may rarely occur, so caution is needed. This medication does not completely cure genital herpes. If symptoms are present during treatment, sexual contact should be avoided. Safe sexual practices are recommended, and treatment should begin immediately if recurrence symptoms appear. No special effects have been reported when driving or operating machinery, but caution should be exercised if symptoms such as dizziness or drowsiness are experienced during treatment. Patients with slightly impaired liver function can take this medication without special dosage adjustments, but those with severe liver impairment should be careful as the effectiveness of this medication may be reduced. This medication has minimal interactions with other drugs, but if taken with certain medications, blood concentrations may change; therefore, consulting with a doctor or pharmacist is advisable. Pregnant or nursing women should take this medication only when absolutely necessary and under the guidance of a physician, as its safety has not been fully established. Children under 18 years old should not use this medication as its safety and efficacy have not been established. Generally, elderly patients do not require dosage adjustments, but if kidney function is decreased, adjustments may be necessary. In case of overdose, seek immediate medical attention for appropriate treatment, as blood dialysis can reduce drug concentration.
